Position Summary:

The Community Case Management Program at Veterans Place provides up to six months of community-based case management for veterans who have recently transitioned into permanent housing. The Community Case Manager provides in home visits with the veterans, supporting them with to set and track personal goals, sustain a healthy lifestyle, and progress toward independence and retaining permanent housing.


Position Responsibilities:

Service Activities:

  • Conduct assessment interviews with potential Veteran clients.
  • Assist Veterans with the creation and implementation of Individualized Service Plans (ISP) which incorporate housing retention such as, employment/income and financial goals, landlord tenant relations, individualized crisis management and development goals.
  • Track Veterans' progress and modify ISPs at minimum, as grant guidelines require.
  • Assist Veterans in accessing ISP goal-based services in the region.
  • Assist with coordination of resources within the continuum of care.
  • Provide in home assessments, meetings and referrals as needed.
  • Facilitate housing search with Veteran clients. (when applicable)

Program Development:

  • Perform case finding functions and outreach, identifying and screening veterans for case management needs.
  • Coordinate supportive services with applicable providers including the Veterans administration.
  • Ensure that Veterans Place services are utilized as part of the provider network along with other Community providers.
  • Oversee the delivery of services within the scope of this program and make recommendations to The Veterans Resource Center Manager.
  • Work closely with the VA Grant and Per Diem Liaison to ensure a high quality of services.
  • Development and implementation of grant/permanent housing retention related group weekly.

Communication:

  • Prepare reports as requested by other Veterans Place and Veterans Administration staff.
  • Participate in administrative, staff, clinical, and in-service activities and training as required.
  • Prepare written reports and complete documents as requested.
  • Communicate policies and the rationale behind them to residents.
  • Inform other agency staff members of relevant matters related to the program.

Education and Experience Requirements:

  • Bachelor's Degree in social service field or equivalent experience
  • Must be proficient in Microsoft Office Suite including Word, Excel and PowerPoint
  • Experience working directly with the homeless population strongly preferred
  • Experience working with Veterans Administration preferred
  • Must have a valid driver's license and be able to obtain automotive insurance
  • Veteran Status is a plus
  • A pre-employment drug and alcohol screening is required, and a background check will be conducted
