We are looking for a motivated, organized, and talented Wellness Navigator to join our team, focusing on playing a crucial role in helping individuals overcome barriers to healthcare access and achieve optimal health outcomes via the Virtual Weight Management (VWM) program. Job Purpose: Responsible to support the growth and carry out clinical administrative functions of the Medical Virtual Weight Management (VWM) Program.

Responsibilities:

  • Works closely with Sales, Clinical Operations & Clinical Leadership to ensure new Virtual Weight Management (VWM) patients are set up correctly in Elation (company's EMR) and company's proprietary system - Community Wellness Connect. This will include patient charts, notes etc.
  • Maintains and updates various spreadsheets to track patient medication order and reorder dates
  • Performs various administrative tasks including but not limited to; interacting with patients to gather necessary information, works with multiple online pharmacies to order & re-order patient medication, reviews enrolled Community Wellness patients to determined eligibility in Virtual Weight Management Program
  • Utilizes the proprietary telehealth system and dashboard to effectively and efficiently support the Virtual Weight Management program
  • Communicates with patients daily via text, phone, engages in patient education, answers patients questions regarding medication, and more as assigned.
  • Utilizes our proprietary EHR (electronic health record) to track all patient data and information to communicate both with patients and co-workers; schedules patient appointments and manages schedules as required.
  • Sets personal tasks and calendar reminders to respond to various patient-specific issues; coordinates schedule with other Clinical Coaches for coverage when needed.
  • Performs all duties on an agreed-upon schedule as assigned by the supervisor.
  • Participates in ongoing, internal, and external training to improve health coaching skills.
  • Performs other duties as assigned.

Here are our MUST HAVES:

Required Education, Experience, Certifications / Licensure, & Skills:

  • Associates Degree in Nursing or Registered Nursing Diploma from a two year college or technical school and three to five years related experience OR the equivalent combination of education and experience.
  • Registered Nursing license in the state of Kentucky or another compact state
  • At least three or more years of previous experience working in a clinical setting
  • Understands remote patient monitoring, telehealth, care/case management, chronic disease
  • Excellent communication skills, both verbal and written
  • Compassionate and non-judgemental when dealing with clinically obese patients/possesses a high level of bariatric sensitivity
  • A planner with capable organization and prioritization skills
  • Able to work well independently and within a team setting
  • Strong interpersonal and relationship building skills
  • A caring and empathetic personality
  • Easily facilitates the exchange of information
  • High attention to detail and accuracy
  • Faster learner with the desire for advanced training

Preferred Experience and Skills:

  • Certification in Health Education or Health Coaching
  • Bilingual (specifically in Spanish) a plus

Requirements:


Job Type (FLSA Status): Non-Exempt

Pay: $45,000.00 to $55,000.00
