Reports To: Clinical Team Leader of Palliative Care
Status: Exempt (Salaried)
FT, PT, PRN: Full-time
Shift: 40 hours per week - 4 weekdays at 10 hours per shift
On Call Rotation: Some nights and weekends phone call ( if included in contract)
Location: Pardee Hospital, Henderson County. Mileage Reimbursement Included.
Position Summary:Provide patient care consultation and treatment to patients with serious illness. Provide teaching and mentoring to team of NP/PA’s within the Palliative Care team.
Essential Duties & Responsibilities:- Provide direct client care consultations and management of symptoms related to life-limiting conditions as requested
- Obtain and review client history
- Assess clients for pain and symptom management
- Prescribe necessary medication for pain and symptom management
- Take calls for palliative care/hospice clients and may serve as attending for hospice clients
- Provide medical aspects of care in collaboration with the interdisciplinary team of attending physicians, nurses, social workers, and chaplains
- Actively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of care
- Create and maintain accurate patient records
- Teach and mentor outpatient and inpatient palliative care providers to obtain quality care outcomes, efficiently with and excellent patient and family experience
- Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
- Practice safety and infection control methods
- Participate in the coordination of the patient’s care in accordance with professional standards of practice
- Participate in the development and evaluation of the plan of care
- Contribute to patient and family counseling and education
- Educate providers in the community on Palliative Care and Hospice services, and end of life issues
- Serve as clinical resource for staff of Four Seasons in addition to staff at facilities served
- Participate in clinical call rotation
- Maintain current medical education and appropriate licensure
- Practice safety and /or infection control methods
- Perform duties in compliance with Medicare Condition of Participation
- Perform other duties as requested or assigned
- Knowledge of HIPAA privacy rules and regulations
- Knowledge of electronic records systems
- Excellent attention to detail and accuracy
- Proficient in MS Office: Outlook, Word, Excel, etc.
