Guardianship Coordinator

The Guardianship Coordinator will be responsible for completing components of the guardianship process as outlined by Wayne Center. Services may be provided in consumer homes, licensed and/or unlicensed settings as well as at various locations throughout Wayne County as contracted by the DWIHN.
Maintains guardianship hearing database
Petitions Wayne County Probate Court for appointment of guardianship
Ensures guardianship packet is sent to probate court
Sends out Notice of Hearing to all responsible parties
Sends out notifications to Legal News
Testifies in court
Responsible for all guardianship modifications
Responsible for receiving a copy of guardianship paperwork distributed in court and/or to consumers and families, guardians and so forth.
Coordinates Informed Consent Board (ICB) hearings for all guardianship petitioners, holding ICB hearings no more than 30 days from receipt of the request for ICB hearing.
Completes ICB packets, forwarding them to Assistant Director of Programs for co-signature, within 5 business days of each ICB meeting.
Distributes completed ICB packets to responsible Supports Coordinator within 3 business days of receiving the signed packet back from the Assistant Director of Programs.
Maintains a small caseload (including transfer intakes, adults and children as needed
QualificationsThe Guardianship Coordinator must have earned at least a Bachelor’s degree in Social Work, Psychology, or Guidance and Counseling or other related human service field from an accredited college or university.
Licensed preferred as a Licensed Master Social Worker (LMSW, Licensed Professional Counselor (LPC), or Limited License Psychologist (LLP) without censure or licensure conditioned (i.e., probationary status).
Have basic computer skills; proficiency with Microsoft Word and the internet required.
Priority will be given to individuals who are credentialed (or are eligible for credentialing) as a Qualified Intellectual Disability Professional (QIDP) and/or as a Children’s Mental Health Professional (CMHP).
Adjusted hours and travel may be required in executing the duties and responsibilities of this position, e.g., travel to homes, schools, vocational settings, hospitals, medical offices, and other community locations as needed.
Extensive scheduled and non-scheduled travel is an essential function of this position. Mileage is reimbursed as specified in the terms of the CBA. Employees are expected to provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle insurance in compliance with State of Michigan requirements are all continuing conditions of employment. In the event, the employee must provide a replacement form of transportation; they are responsible for assuring compliance with these directives.
Experience working with children and adults with Intellectual and Developmental Disabilities performing assessments, testing, and evaluation for a minimum of 2 years post-degree is required.
Experience working within the MHWIN system or Electronic Health Record for a minimum of 1 year is preferred.
Experience performing Intake Assessments for a minimum of 1 year is preferred.
