InnerSourced Solutions is seeking to immediately hire a part-time, 1099 contractual Female Rehabilitation Coordinator to assist child and adolescent clients with learning and developing improved daily living skills (i.e. personal hygiene, organizational skills, time management skills, nutrition, and money management), as well as working towards achieving healthy coping mechanisms.  Services are provided in the home, community, school, and office settings, as determined by the needs and/or desires of the client. This position will be serving youth between the ages od from 8 - 17 years old. Priority will be given to candidates who can complete the on-boarding process and training within 14 days.


RESPONSIBILITIES:


  • Meet with assigned clients 4 times per month;
  • Perform rehabilitation activities at the office, in clients’ homes, in the community, or remotely if appropriate;
  • Develop client specific, culturally appropriate intervention strategies including goal setting, planning, and decision making;
  • Complete and submit thorough, accurate, and timely documentation including in encounter notes, Individual Rehabilitation Plans, discharge/transition recommendations and summaries; in accordance with agency policy and all applicable local, state, and federal regulations;
  • Maintain consistent contact with clients and work cooperatively with other members of the client’s treatment team;
  • Uphold with high regard, the confidentiality of clients and follow all HIPPA guidelines.


QUALIFICATIONS/ REQUIREMENTS:


  • Associates or Bachelor’s degree in a mental health or human services related field; OR 30 hours or equivalent of college credit towards a Bachelor’s degree in a health related field; OR a High school diploma with 1 year of work experience in a supervised mental health setting 
  • Must possess or obtain certification in First Aid and CPR.
  • Must pass state and Child Protective background checks.
  • Active, unrestricted Maryland driver’s license & reliable personal vehicle.
  • Must reside in the state of Maryland or within a sustainable driving distance for monthly on-site visits.
  • Must be able to provide on-site client services once per month as we provide a blended service/
