Nurse Care Manager (Bilingual - English/Spanish)
Position Overview:
Join a dedicated team focused on improving the lives of Medicare-eligible Hispanic seniors through compassionate care and advocacy.
Key Responsibilities:
- Manage chronic care and transition processes for high-risk patients.
- Collaborate with a multidisciplinary care team to develop and implement care plans.
- Perform assessments to identify and address physical, mental, and social health needs.
- Ensure smooth transitions from acute to post-acute care settings.
- Coordinate follow-ups to prevent readmissions and enhance patient care progression.
- Utilize data to inform care decisions and participate in performance improvement initiatives.
- Document all patient interactions in the Electronic Medical Record accurately and promptly.
Qualifications:
- Minimum 2 years of RN experience, preferably in case management or chronic care management.
- Strong interpersonal, organizational, and communication skills.
- Familiarity with chronic care management, Medicare systems, and performance improvement.
- Bilingual (English/Spanish) preferred.
- Bachelor's degree in nursing or healthcare-related field; Master's preferred.
- Active RN License (Texas or Multi-state Compact).
Working Conditions:
Full-time role during clinic business hours.
