Job Summary

This position is responsible for working directly with youth in care providing care coordination, referral services, and assisting youth in developing goals related to their individualized service plan. The Care Coordinator will coordinate with the Loft Team, and Leadership regarding youth care and collaborate with guardians, case workers, and other community partners regarding their care plan. 


Job Duties/Responsibilities

To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. This job may include any of the responsibilities listed below:

 

  1. Rotate with leadership being on-call after-hours approximately 1 week each month to provide guidance on Loft referrals and other related emergent matters. When appropriate, authorize readmission of youth who have run away from the HOPE center. The basis for readmission must be documented by specific factual findings.
  2. Meet with a youth within eight hours of their Loft admission.
  3. Complete intakes and assessments of youth to determine current needs, legal status, and housing barriers to create an individualized service plan.
  4. Facilitate the youth’s return to their legally authorized residence (including DCYF in the case of a dependent youth) as soon as possible or initiate processes to arrange legally authorized appropriate placement.
  5. Interface with other relevant resources and system representatives to secure long-term housing placement and other needed services for the youth.
  6. Facilitate a physical examination for youth within the timeline required by licensing.
  7. Facilitate an educational assessment for youth to measure the youth’s competency level in reading, writing, and basic mathematics. The assessment must measure learning disabilities or special needs.
  1. Represent our youth services in the community to develop partnerships that strengthen the program and advocate for systematic change that improves the lives of youth experiencing homeless and their families affected by substance use disorders and mental health disorders.
  2. Works in collaboration with Loft staff and other partners in providing care coordination to stabilize program youth who exhibit significant healthcare challenges, including substance use disorders and mental health disorders, facilitate an evaluation as necessary with a Designated Crisis Responder.
  3. Work directly with youth to meet their goals and provide supportive listening, teach daily living skills, teach mindfulness skills, and promote activities/skills which support mental wellness.
  4. Master youth services electronic records system via Salesforce documenting all youth related services.
  5. Provide aftercare follow-up services to youth who have exited the program and support community outreach to maintain relationships with the youth in our community.
  6. Provide leadership within the Loft by modeling professional boundaries--aligning with Catholic Charities Code of Conduct. Provide direct, transparent, and respectful communications.
  7. Ensure client confidentiality according to Catholic Charities policies and procedures.
  8. Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  9. Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
  10. As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
  11. Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.
  12. Perform related functions necessary to support the mission and core values of Catholic Charities.


Job Qualifications

To perform this job successfully, an individual must meet the minimum qualifications listed below.  These qualifications are representative of the knowledge, skill and/or ability required to perform this job. 

 

Education/Experience: To perform this job successfully, an individual must have: a Master’s degree in social services, or a closely related field from an accredited school and one year of experience working with children or youth, or a bachelor’s degree in social services work or a closely related field from an accredited school and two years of experience working with children or youth; knowledge and/or experience in providing case management services; experience in direct client services.  Experience working with issues of homelessness and poverty preferred.

 

Certificates/Licenses:  To perform this job successfully, an individual must have a valid Driver’s License and the ability to drive for work use. Successfully pass background check applicable to position.


Physical Abilities:  To perform this job successfully, an individual must be able to:

  • Regularly sit, stand, climb, walk, hear/listen, talk.
  • Frequently lift to 25 pounds, pull/push, carry, grasp, reach, stoop, kneel.
  • Clearly see 20+ feet, with or without corrective lenses, ability to focus.
