Crystal Home HealthCare is looking for PT Case Managers to join our team of healthcare professionals and make a difference, and we’re inviting you to be part of our team  We have open positions and are looking for the very best to support clients in their homes in the Metro Detroit Area, specifically Southern Oakland and Wayne County.


Qualifications

  • One year of PT experience in a Skilled/Clinical environment
  • Current State of Michigan Physical Therapist (PT) license in good standing
  • Graduate with Bachelor of Science or Master of Science Degree, in Physical Therapy, Doctorate of Physical Therapy.
  • Able to provide clinical services in the home and able to instruct/teach patient and caregivers
  • Able to supervise PTAs and home health aides providing services to the patient according to regulatory guidelines and Physician Supervised care plan
  • Current & Valid Driver’s License with reliable and insured transportation
  • Current CPR Certification
  • Current Negative TB test/negative chest x-ray
  • Functioning Electronic Device with cellular service so you can chart in the field
  • Knowledge and experience working with home health care specific Electronic Charting
  • Work requires knowledge of the Homecare Industry, Medicare/Medicaid/CMS/State of Michigan regulations
  • Freedom from illegal use of drugs.  Freedom from use and effects of the use of drugs and alcohol in the workplace.
  • Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a healthcare-related setting are ineligible for employment in the position.

 

Essential Functions:

  • Care for multiple patients throughout the day in their individual home environments
  • Evaluate patients to determine type/frequency of therapy treatments needed
  • Evaluate joint motion, muscle strength, patient endurance to ensure patient can participate in ADLs
  • Provide home exercise program and instructions to ensure improved clinical outcomes
  • Provide functional training such as use of assistive devices, transferring techniques and ambulation improvement
  • Perform and complete comprehensive assessments including CMS OASIS document
  • Report significant findings to physician and Clinical Manager
  • Implement physician orders in a timely, complete and accurate manner
  • Ensure the safety of the treatment procedures, patient and environment at all times
  • Supervise and evaluate the care provided by PTAs and aides
  • Teach the patient and/or family members about new diagnosis, care, and support skills
  • Submit documentation in a timely, complete, and accurate manner
  • Attend in-services and meetings as required
  • Maintain HIPAA compliance and patient confidentiality at all times
  • Position requires 100% travel to our patient’s home
