The pharmacist in charge position provides daily oversight and coordination of all pharmacy-related activities at UI Community HomeCare Pharmacy. Specifically, the role provides and manages clinical evaluation and monitoring of therapies, sterile compounding, and home infusion dispensing services for UI Community HomeCare patients.
This position is exciting and unique as it plays an integral role for our UI Community HomeCare team; you will work closely with other departments such as our nursing and clinic support teams as well as our leadership team. This position will be 100% full time. Office hours are Monday through Friday 8 am to 5 pm. The office is closed on holidays and weekends.
A. Patient Care - Participate in the provision of pharmaceutical care in the pharmacy
- Provides therapeutic drug monitoring for home infusion therapies.
- Evaluate patient-specific medication regimens to ensure the appropriateness of the following:
a. Indication
b. Correct dose
c. Route of administration
d. Drug-drug, drug-food, drug-lab, or drug-disease state interactions
e. Efficacy of therapy
f. Adherence to therapy
g. Dose adjustments for organ dysfunction
h. Adverse drug reactions
i. Potential drug misuse or abuse
j. Use of the most cost-effective therapy
3. Complete a comprehensive patient assessment including medication history, pertinent physical assessment, assessment of relevant laboratory values, patient interview, and an evaluation of past medical history for appropriate patients.
4. Confirms/clarifies all patient drug allergies noted in the medical record.
5. Work with physicians, nurses, pharmacists, and other health care providers as multi-disciplinary team members.
6. Provide in-depth patient medication education.
7. Identify, evaluate, and prevent adverse drug events through appropriate interventions.
8. Document relevant clinical information in the electronic medical record.
9. Collaboratively address financial issues to ensure receipt of needed medications.
10. Provide drug information and education to health care professionals.
B. Provide daily communication and follow-up to other clinical pharmacists, technician staff, UI HomeCare areas, and other health care professionals as needed.
- Provide in-service education to pharmacists as needed.
- Provide in-services to members of the health care team as needed.
- Assist physicians, nurses, and patients with medication questions, medication access, and continuity of care issues.
C. Documentation of patient care activities:
1. Medication errors in the standard reporting system.
2. Adverse drug reactions into the standard reporting system.
3. Patient allergies and height/weight information as needed.
4. Medication counseling is provided to patients.
5. Interventions with prescribers and other health care providers.
6. Follow-up telephone encounters.
7. Immunization history.
8. Medication list.
9. Others as required.
D. Administration
1. Coordinate therapeutic decision-making when there is a paucity of literature or experience for reference.
2. Identify and resolve significant/highly unusual patient drug therapy problems.
3. Manage and analyze medication error–related events, including participation in informal multi-disciplinary sentinel event review teams.
4. Adhere to all department policies and procedures.
5. Contribute to the department budgetary process by reviewing, designing, implementing, and monitoring budget initiatives that address specific issues.
6. Participate in implementing and monitoring new programs developed either by the UI HomeCare or through other means. (e.g., Leadership Team)
7. Participate in ACHC accreditation and Iowa Board of Pharmacy surveys.
8. Serves as Pharmacist-In-Charge for Iowa Board of Pharmacy.
9. Plan for and supervise operational activities of the home infusion pharmacy. Maintain proper inventories and staffing.
10. Hire, develop and manage the performance of staff, assure staff is compliant with policies and procedures, laws, and regulations.
11. Mentor/train staff, and provide feedback, counseling, and disciplinary action when warranted. Perform annual reviews. Maintain employee schedules.
12. Compliance with Internal policies and external Regulations: Establish and maintain procedures and systems to assure compliance with state and federal laws.
13. Develop and implement and monitor the QA plan. Initiate corrective action as needed.
E. Quality Improvement
1. Provide a leadership role involved with drug therapy and pharmacy committees.
2. Participate in measurement activities that demonstrate pharmacy value, including ADRs and outcome measures.
3. Perform quality assurance monitoring of services provided by UI Community HomeCare.
F. Other

Education Requirements
- Bachelor of Science degree in Pharmacy or Doctor of Pharmacy degree is required.
- Current eligibility for a pharmacist license by the Iowa Board of Pharmacy Examiners is required.
- 3 years of experience in pharmacy is required.
- Ability to manage complex projects is required.
- Excellent written and verbal communication skills are required.
- Job-related experience with and/or commitment to service excellence is required.
- Job-related experience with and/or commitment to diversity in the work environment is required.
Desired Qualifications
- Satisfactory completion of an ASHP-accredited residency is desirable.
- Job-related experience with sterile compounding is highly desirable.
- Pharmacy manager experience is highly desirable.
- Job-related experience with HIT services and billing is highly desirable.
- Brighttree experience highly desirable.
