New Paths, Inc. is looking to fill a full-time, non-exempt position for an SUD Home Health worker. See description below.

Job Summary: The SUD Home Health worker works with clients as an advocate and acts as a liaison in providing appropriate community resources. The SUD HH will identify and gather community resources, observe/participate in client assessments and action plan.

Supervisory Responsibilities:

  • None

Duties/Responsibilities:

  • Support and adhere to the NPI Code of Ethics, work rules, policies, plans and procedures to promote a safe and secure environment for the residents, visitors and employees of New Paths.
  • Identify and gather appropriate community resources, including but not limited to mental health, SUD, medical (health education), housing, financial resources.
  • Provide problem-solving techniques with clients.
  • Meet with and communicate with clients regularly.
  • Participate in health promotion activities or health education class
  • Observe/participate in client assessments.
  • Assist in implementing client action plans.
  • Develop and maintain education materials as resources for staff and clients.
  • Develop and maintain cooperative working relationships with agencies and organizations in the community.
  • Document activities and record information.
  • Other duties as assigned.

Required Skills/Abilities:

  • Excellent interpersonal and customer service skills.
  • Ability to interact professionally and respectfully with staff and clients.
  • Excellent verbal and written communication skills.
  • Proficient in Microsoft Office Suite or related software.
  • Ability to work independently.
  • Ability to coordinate and effect a smooth flow of behavioral-based information and data
  • Skill in communicating with a variety of people from varying backgrounds
  • Excellent organizational skills, including the ability to deliver information and services in a timely fashion. Time management.
  • Exhibit problem solving and conflict resolution techniques.

Education and Experience:

  • Minimum of 3 years’ experience in health services field required.
  • Experience in SUD settings preferred.
  • CHW Certification, preferred.
  • High School Diploma or GED

Physical Requirements:

  • Prolonged periods sitting at a desk and working on a computer.
  • Periods of walking throughout building.
  • Ability to climb stairs as necessary.
  • Must be able to lift up to 15 pounds at times.
