Job DescriptionJob DescriptionNew Paths, Inc. is looking to fill a full-time, non-exempt position for an SUD Home Health worker. See description below.
Job Summary: The SUD Home Health worker works with clients as an advocate and acts as a liaison in providing appropriate community resources. The SUD HH will identify and gather community resources, observe/participate in client assessments and action plan.
Supervisory Responsibilities:
- None
Duties/Responsibilities:
- Support and adhere to the NPI Code of Ethics, work rules, policies, plans and procedures to promote a safe and secure environment for the residents, visitors and employees of New Paths.
- Identify and gather appropriate community resources, including but not limited to mental health, SUD, medical (health education), housing, financial resources.
- Provide problem-solving techniques with clients.
- Meet with and communicate with clients regularly.
- Participate in health promotion activities or health education class
- Observe/participate in client assessments.
- Assist in implementing client action plans.
- Develop and maintain education materials as resources for staff and clients.
- Develop and maintain cooperative working relationships with agencies and organizations in the community.
- Document activities and record information.
- Other duties as assigned.
Required Skills/Abilities:
- Excellent interpersonal and customer service skills.
- Ability to interact professionally and respectfully with staff and clients.
- Excellent verbal and written communication skills.
- Proficient in Microsoft Office Suite or related software.
- Ability to work independently.
- Ability to coordinate and effect a smooth flow of behavioral-based information and data
- Skill in communicating with a variety of people from varying backgrounds
- Excellent organizational skills, including the ability to deliver information and services in a timely fashion. Time management.
- Exhibit problem solving and conflict resolution techniques.
Education and Experience:
- Minimum of 3 years’ experience in health services field required.
- Experience in SUD settings preferred.
- CHW Certification, preferred.
- High School Diploma or GED
Physical Requirements:
- Prolonged periods sitting at a desk and working on a computer.
- Periods of walking throughout building.
- Ability to climb stairs as necessary.
- Must be able to lift up to 15 pounds at times.
