Make a difference as a Licensed Master Social Worker

As a new Licensed Master Social Worker at Sparrow Hospice, you will be responsible for conducting assessments, providing counseling and support services, and collaborating with interdisciplinary teams to develop care plans. Your day-to-day tasks may include conducting home visits, connecting patients with community resources, and advocating for their needs. You will also be expected to maintain accurate records, participate in team meetings, and stay up-to-date on best practices in the field.

This role requires strong communication skills, empathy, and the ability to work effectively in a fast-paced environment while maintaining a high level of professionalism.

What you need to be successful
  • To excel in the role of Licensed Master Social Worker at Sparrow Hospice, candidates must hold a valid LMSW license in the state of employment and have a master's degree from an accredited school of social work.
  • A minimum of one year of social work experience in a healthcare setting is required, with preference given to candidates with Medicare home health or hospice experience.
  • The successful candidate will demonstrate the ability to make independent decisions based on client needs and prioritize multiple critical tasks effectively.
  • Strong communication skills, empathy, and the ability to work collaboratively within an interdisciplinary team are essential for success in this role.
