OVERVIEWThe Lucy Daniels Center is a growing 501(c)(3) nonprofit organization where children’s mental health is the priority. Founded in 1989, Lucy Daniels Center is the largest nonprofit provider of social, emotional, behavioral, and mental health services exclusively for children in the Greater Triangle area. We serve more than 800 children and their families yearly and provide more than 40,000 hours of direct social, emotional, and mental health services to children from birth to age 11, the time of life when the most effective interventions are possible.The multidisciplinary team, which includes recognized experts in mental health and education, works together with families to provide a uniquely comprehensive approach, delivering individualized treatment to each child. LDC’S SERVICES INCLUDE:
  • Therapeutic classroom settings for preschool through Grade 5 children at the Lucy Daniels School.
  • Farley-Manning Family Guidance Service, providing onsite and telehealth therapy for children and families.
  • SecurePath, a comprehensive in-home mental health service for children birth-5 years old. SecurePath is delivered in conjunction with Wake County SmartStart’s initiative to ensure that emotionally at-risk young children have early intervention services to better prepare them for success in school and life.
  • Lucy’s Link, providing observations and assessments for young children in the community.
  • Resources and Support, Lucy Daniels Center skilled staff provide parenting groups, social skills groups, observations and assessments, caregiver workshops and support groups, and teacher training across North Carolina in partnership with organizations like Telamon Corporation and the Psychoanalytic Center of the Carolinas.

THE POSITION SUMMARYSecurePath is a bilingual (English, Spanish) early-intervention program for children from birth through five years old where services are provided either in their home or pre-school setting. Our SecurePath team consists of mental health clinicians who provide early intervention mental health treatment for children experiencing emotional or behavioral issues. SP therapists work closely with both child and caregiver, typically in the context of the family’s home environment. Most of the families served in the SP program require case management support along with mental health services. SecurePath is funded by LDC’s partnership with Wake County Smart Start, which works to support Kindergarten readiness.The eligibility for the SecurePath program is based on household income, insurance status, or other “greatest need” measures. This is currently a temporary part-time position of 15 hours per week through June 30, 2023; if funding for the position continues for the next fiscal year, there is potential for the position to grow. This position is a hybrid blend of remote and in-person work, as some SecurePath families have technological barriers that require their case management support to be done in person. It may be beneficial for the Case Manager to attend most new intakes to do a case management assessment.The successful candidate is highly organized, and able to take initiative, working autonomously and with the team. The successful candidate can operate in a fast-paced environment while remaining good-natured, resourceful, and efficient. A high level of maturity, integrity, professionalism, discretion, sound judgment, tact, confidentiality, and self-care are crucial. The role will transition to full-time (40 hours per week) starting July 1st, 2024.

  • Assess case management needs with new families.
  • Refer families to other agencies to receive services.
  • Help caregivers apply for housing, food stamps, diapers, clothing, etc.
  • Make calls for medical appointments as needed.
  • Help caregivers navigate job applications.
  • Participate in outreach and networking in the community.
  • Join weekly individual calls with team members and join weekly team meeting.
  • Attend whole-staff meetings.
  • Participate in Center activities as requested.
  • Other duties as assigned.

SKILLS, QUALIFICATIONS, AND ATTRIBUTESThe Case Manager must demonstrate a passion for and willingness to support The Lucy Daniels Center’s mission, vision, values, and work. The ideal candidate will possess sufficient experience to execute the responsibilities of the position and demonstrate the following:
  • Bilingual Spanish and English speaking (required).
  • Bachelor’s degree in healthcare management or related field including social work, nursing, or health and human services (required).
  • Strong verbal and written communication skills (required).
  • Self-directed work style (required).
  • Organizational skills and ability to multitask (required).
  • Proactive; internal drive to continuously and successfully meet goals (required).
  • Proficient with technology, including Microsoft applications and Zoom (required).
  • Previous case management experience. Employees must have a health registry check in accordance with 10 NCAC 27 G.0200. and a criminal record check with fingerprinting (required).
  • Experience working with children and families in case management or other capacities (preferred).
  • Experience in mental health field in some capacity (preferred).
  • Knowledge/background navigating Wake County Public School system or similar IEP systems (preferred).
  • Understanding and appreciation of diversity, equity, and inclusion, and desire and humility to grow and learn with the team.
  • Willingness to extend kindness, compassion, and accountability to oneself and others.
  • Proficient with computer skills and knowledge of database programs.
  • Willingness to operate in compliance with organizational policies and procedures and external (donor/legal/IRS) requirements.

PRIMARY RELATIONSHIPSThis position will work closely with our SecurePath mental health clinicians. This position reports directly to the SecurePath Senior Team Lead and will also interact routinely with the other staff members at LDC.
