👩️‍⚕️ Home Health Nurse - Tolland, CT 06084

📍 Location: Tolland, CT 06084
💵 Salary: $40 - $50 per hour

Join our compassionate team as a Home Health Nurse, providing expert nursing care to patients in their homes in Tolland, CT.

Responsibilities:

  • 🩺 Perform comprehensive patient assessments and develop care plans.
  • 💉 Administer medications and conduct treatment as prescribed.
  • 🌡️ Monitor patient conditions and adjust care plans as necessary.
  • 📋 Maintain precise medical records and documentation.
  • 👥 Work collaboratively with healthcare professionals to ensure complete patient care.
  • 📚 Educate patients and their families on health conditions and care requirements.

Qualifications:

  • 🎓 Nursing degree with active RN or LPN license in Connecticut.
  • ❤️ Current CPR/BLS certification.
  • 🏡 Prior home health experience is a plus.
  • 💻 Proficiency with EMR systems.
  • 🤝 Strong interpersonal skills with a compassionate bedside manner.
