IDT Coordinator

The IDT Coordinator will be responsible for the daily coordination of the IDT tasks in alignment with regulatory requirements. Coordination includes supporting facilitation of the IDT daily meeting and Plan of Care (POC) meetings, ensuring tasks are completed to maintain compliance for Service Determination Requests (SDR), scheduling POC meetings within regulation, and ensuring appropriate assessments are completed for required POC updates and change in condition needs.


What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 163 PACE programs, serving more than 72,000 participants in 32 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

What You Bring:

  • High school diploma or equivalent, associate degree is preferred.
  • Minimum of 1 year of experience working (paid or volunteer) with the elderly preferred.
  • Prefer experience to being in a geriatric care setting.
  • Experience in a PACE program is highly desirable.
  • Excellent interpersonal and communication skills including meeting facilitation.
  • Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
  • Effectively work with and build relationships with others.
  • Excellent computer and technology skills.
  • Excellent organizational skills.
  • Highly detail oriented.
  • Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
  • Ability to work collaboratively and communicate effectively
