Job Title: Developmental Disability Professional (DDP)
About Us: American Hearts & Home is committed to providing top-quality services to individuals with developmental disabilities. We are seeking a dedicated Developmental Disability Professional (DDP) to join our team.
qualification Requirements:
- Registered Nurse (Associate Degree or Diploma) in Nursing or human services field including but not limited to: sociology, special education, rehabilitation counseling, and psychology AND has one year of experience in providing human services to
individuals with I/DD. - Proficient in Microsoft Office
Key Responsibilities:
As a Developmental Disability Professional, you will be responsible for:
- Overseeing the delivery of waiver and state services to participants with a focus on overall quality of service delivery by the provider agency.
- Providing general guidance to the provider agency in areas of compliance and quality improvement.
- Ensuring that supports provided are within the scope of the agency’s service enrollment and experience.
- Addressing participant’s needs while adhering to person-centered values, choice, and participant’s rights.
- Providing, arranging, or overseeing curricula used in staff training in alignment with individual goals and objectives.
- Recommending other needed services/supports or changes to the delivery model using a continuous quality improvement approach.
- Offering consultation to the provider agency in ISP implementation strategies.
- Assessing areas of risks and providing risk mitigation strategies.
- Reviewing functional assessments and ensuring they support the participant’s plan for service delivery.
- Overseeing high-intensity services addressing health and safety risks.
- Supervising the formulation of the individual’s plan for service delivery.
- Reviewing Therap documents and reporting any issues to the office administrative staff and supervisor(s).
Training Requirements: In addition to the initial orientation requirements, DDPs are required to complete the following trainings in their first year of employment:
- Individual Service Planning (Person-Centered)
- Support Intensity Scale overview
- Health Risk Screening Tool online training overview
Documentation Requirements:
- Ensure all necessary face-to-face participant visits and contacts are documented.
- Maintain documentation of date, location, signature (title), and time of service delivery.
Additional Requirements:
- Must be able to purchase own paid trainings such as CPR, CPI, COGENT Report, etc.
- Must have a valid GA driver’s license, good driving record, and access to a reliable vehicle.
- Must be able to work flexible hours to meet individual and program needs.
- Must be able to lift at least 40 lbs.
Job Types: Part-time, Full-time
Pay: $18.06 - $21.75 per hour
Schedule:
- 8 hour shift
Work Location: Hybrid
