Registered Nurse Needed

The Registered Nurse (RN) is a key member of the A & C Private Homecare team, managing the day-to-day service delivery to members participating in Home Care programs. Full-time position with a flexible schedule and fieldwork.

REPORTS TO

Reports directly to the Branch Manager with additional supervisory support from the Director of Nursing

JOB DESCRIPTION

• Desire and capacity to work with elderly, frail, and disabled populations.
• Supervise all services delivered Home Care
• Develop a Care Plan for members. Review and update Care Plan annually and/or as needed.
• Become familiar with compliance requirements found in CCSP General Part II and Personal Support Chapters.
• Follow CCSP clinical guidelines and A & C Private Home Care Policies and Procedures when providing member care and oversight.
• Conduct on-site skills check offs and training of PSAs.

Maintain Employee files (Quarterly in-services (8hrs annually), Host CPR classes, and Administer TB tests.

Oversee Client Chart audits.

Maintain client care logs.

• Conduct supervisory visits to members' homes where the Care Plan is reviewed and revised, as necessary.

Conduct Initial and Quarterly Assessments

• Administer and evaluate TB skin tests to all employees when hired and annually
• Be available via phone after normal business hours and on weekends to answer any questions or emergency situations PSAs may encounter while providing care in members' homes.

MINIMUM QUALIFICATIONS

• Current Georgia RN license
• Current first aid and CPR certification
• General proficiency with PC software and web-based applications and databases

PREFERRED QUALIFICATIONS

• Familiarity with Home Care program or comparable care protocols

Geriatric care

• Program management experience Examples of Home Care Admitting Nurse Duties:
• Evaluate home conditions
• Review of Comprehensive Care Plan delivered from Care Coordinators
• Train PSAs on member-specific criteria
• Compile medication log
• Assess member's overall health condition
• Communicate with members

Examples of Home Care Supervisory Visit Duties:

• Evaluation of PSA performance of care
• Review and re-assessment of Care Plans
• Conduct member survey to determine satisfaction wit A & C Private Home Care service provision
• Record changes to medication logs
• Take vital signs and assess overall health conditions
• Identify any areas of care that need to be addressed, and develop a plan of correction for physicians on clinical matters
• Train client and family members on self-care
• Complete clinical documentation in a clear, correct, complete, and legible fashion
