BoldAge delivers innovative, creative solutions for PACE (Program of All-Inclusive Care of the Elderly). Our model of care enables older adults to live a meaningful, independent life at home with grace and indignity.

We meet the evolving needs of older adults by providing innovative disciplinary solutions that consider both their physical and mental well-being. Our owner operated model is aimed at expanding the ability for older adults to live happily and healthfully in their own homes for as long as possible.

Position Summary: Under the direction of the Director of Outreach and Engagement (OED), is responsible for generating referrals into the PACE program and educating the community through community outreach, networking, and marketing activities in the community.


Promotes and maintains high-quality services according to BoldAge values, PACE principles and best practices.Develops and cultivates the PACE organization’s network of business, healthcare, and community relationships by· developing a deliberate and organized approach to identifying and engaging with referral partners within the community· actively participating in community groups· presenting at community events about PACE and care of older adults· cultivating and maintaining positive and constructive relationships with healthcare providers, senior service providers and other referral sources· establishing positive relationships with potential participants, their families, and caregivers· responding timely to all inquiries and commitments· evaluating and learning about the needs of the community, especially older adults, and their families, and incorporating learned information into community outreachFacilitating the assessment and eligibility determination processes by identifying quality referrals and collaborating with other enrollment staff and the interdisciplinary team.


Basic QualificationsEducation:A baccalaureate degree in health care administration, business, communications, marketing, or related human services field is preferred.High School diploma or equivalent is required.


Experience:Minimum 1 year experience working with the elderly required.Experience in community outreach and engagement, intake, enrollment, and marketing preferred. Management experience in a human services organization preferred.


Skills and Knowledge:· A “people first” approach to all decision making, planning and program practices.· Knowledge of managed care environment including current managed long term care programs in the community and implications for the PACE program growth.· Communication expertise with staff, colleagues, and clients in a variety of settings, including the ability to communicate clearly and to listen well.· Assertiveness in promoting services that exceed expectations and a culture of “doing the right thing” in all situations.· Demonstrated commitment to promoting quality services to a frail client population with a focus on continuous improvement and a “do it better” approach.· Ability to be creative and innovative in developing systems and making decisions.· Understanding of the principles of diversity and inclusion· Knowledge of community resources, demographics, and characteristics of the service area and services available to seniors and their families.· Ability to implement effective community outreach and engagement strategies that lead to continued census growth.· Skills to effectively represent the program to participants, families, outside agencies, community groups, community professionals and the general public.· Effective relationship-building skills with community members, referral sources and co-workers.· Knowledge of federal, state, and local regulations and requirements related to PACE outreach and engagement practices.· Ability to maintain accurate records and to prepare clear and concise reports, correspondence, and other written materials.· Excellent verbal and written communication skills.· Excellent public speaking skills for large and small groups.· Knowledge of data tracking systems, analysis, and reporting.
