Work Schedule:

  • Weekday 8-hour shifts Monday through Friday.
  • One weekend on-call every 8th weekend, with a day off during the week.
Essential Functions:
  • Schedule and document client schedules, changes, and incidents in a timely manner.
  • Coordinate care with management and client care team.
  • Supervise and manage assigned care team, including annual onsite visits and caregiver evaluations.
  • Complete annual client supervisory visits.
  • Conduct client file audits and rectify discrepancies promptly.
  • Provide effective communication to clients, families, community liaisons, staff members, and other healthcare professionals.
  • Respond to agency and client needs professionally.
  • Demonstrate commitment and competency in all duties.
  • Enhance professional growth through participation in educational programs, staff in-services, and workshops.
  • Promote the agency philosophy and administrative policies to ensure quality care.
Requirements:

Education:

  • High School graduate or equivalent.

Licensure:

  • Current Texas driver’s license.

Experience:

  • One year of customer service experience preferred.
  • Home Care or Personal Assistant Service scheduling experience mandatory.

Skills:

  • Excellent computer skills.
  • Exceptional interpersonal, organizational, and communication skills.
  • Knowledge of medical terminology preferred.
  • Excellent phone skills.
  • Ability to travel locally with some exposure to inclement weather.

Transportation:

  • Reliable transportation and valid, current auto liability insurance.
Environmental and Working Conditions:
  • Routine office environment; noise level may be moderately high.
  • Must be able to work a flexible schedule and extended hours.
  • Participates in the On Call and Back Up On Call Care Coordinators schedule.
  • Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort:
  • Prolonged standing and walking required.
  • Ability to lift up to 50 lbs.
  • Requires working under some stressful conditions to meet deadlines and client needs.
  • Ability to make quick decisions.
  • Excellent problem-solving skills and ability to self-manage.
