Job Summary:

Located in the heart of Maryland’s Mid-Shore region, Choptank Community Health System (CCHS) provides primary health care services in Caroline, Dorchester, Kent, Queen Anne's and Talbot counties. Maryland’s Eastern Shore enjoys the beautiful Chesapeake Bay setting, characterized by historic towns, meandering creeks, beautiful natural areas, Maryland Blue crabs, oysters, and clams. All this, and we’re still just a short trip to Baltimore, Washington, DC, and Philadelphia.

CCHS is seeking a highly motivated and skilled Nurse Practitioner to work in our School-Based Health Center (SBHC) program. School-Based providers for a 12-month position which may work in CCHS health centers and/or Migrant Health program during the summer months.

SBHC provides an effective means for students to access comprehensive health care services. SBHCs provide care in a location that meets students where they are: at school. Multiple studies have shown SBHCs to be a cost-effective investment in child and adolescent health by reducing the utilization of emergency rooms, improving health outcomes, and elevating school attendance and graduation.

Our Joint Commission Accredited system includes seven medical offices, six dental offices, over forty medical and dental providers, behavioral health, school-based wellness, and migrant health programs, for a total of 300 team members. Our mission is to provide access to exceptional, comprehensive, integrated health care for all.

Education and Experience:

Duties/Responsibilities:

  • Comprehensive patient management, using leading edge technology, including EMR.

  • Regular, reliable attendance is a requirement of this job.

Job Type: Full-time

Salary: Negotiable based on experience
