A DAY IN THE LIFE OF AN ADVANTAGE CASE MANAGER RN
The ADvantage (ADV)/State Plan Personal Care (SSPC) home care nurse provides services listed in the person-centered service plan. The services may be provided by a Registered Nurse (RN), or a Licensed Practice Nurse (LPN) under the supervision of an RN, licensed to practice and in good standing in Oklahoma. The nurse promotes Member independence and safety to prevent or postpone the institutionalization of the Member.
QUALIFICATIONS FOR AN ADVANTAGE CASE MANAGER
Oklahoma RN license required.
Home Health, Critical Care or Emergency Room experience preferred.
Requires excellent verbal and written communication skills.
Shows expertise in interpersonal relationships and competence in working with professional personnel and a variety of publics in the community.
Ensures that all necessary supplies and equipment are available.
Assists with other duties or projects as requested by Case Manager Supervisor.
WORK SCHEDULE
This full-time Case Manager position works flexible self-scheduling hours, 50% working remotely and 50% of the time traveling and completing in home member and assessments and monitoring.
