Job DescriptionJob DescriptionDescriptionThe Hospice Triage RN provides nursing assessment, planning and care to maximize the comfort and health of patients and families. The Triage RN responds to all phone calls promptly and appropriately after hours, makes patient visits as necessary and completes after hours admissions and deaths as needed.
  • Remote
  • Friday 5pm- Monday 8am
  • Must have multi-state license
  • Must live in Oklahoma, Texas, Missouri, or Kansas
  • Hospice experience required


Key Responsibilities
  •  Phones Patient Care Manager at the end of each workday to receive report of patient’s status. 
  •  Responds to calls received on page in a timely fashion. All calls will be returned no later than ten (10) minutes from page. 
  •  Communicates with patient/family concerning specific reason for call. If able to handle situation via telephone, document accordingly. 
  •  If requires a visit in the case of a crisis, admission, death, etc., or a visit is requested; will make visit as necessary. Visits must be made within 1 hour of call or family must be made aware of estimated time of arrival. 
  • Documents all phone calls received on-call log. 
  • Maintains appropriate forms and supplies. 
  • Attends IDT meetings, Monday morning meetings, and all required in-services. 
  • Turns in documentation on a timely basis. 
  • Reports to Patient Care Managers at the beginning of each workday as needed. 
  • Maintains confidentiality of patient/family status. 

Skills Knowledge and Expertise
  • Minimum one (1) Year of Hospice Experience Required
  • Multi-state license 
  • Strong Organizational and Self-Management Skills
  • Critical Thinker with Good Judgment
  •  Previous experience in hospice, home health, geriatrics or general medical/surgical nursing. 
  • Demonstrated commitment to the hospice philosophy of care.
  • Commitment to Providing Excellent Customer Service to Patients and Families
  • Valid Driver’s License
  • Can Successfully Pass Background Checks
