Community Care Hospice, a collaboration of several non-profit senior living communities, is seeking a compassionate and dedicated Part-time Licensed Master Social Worker (LMSW) to join our team

Summary:
As a Social Worker, you will play a vital role in providing exemplary end-of-life care to residents in Saline, McPherson, Marion, Reno, and Harvey counties. Partnered with Midland Care Connection, a renown Kansas hospice provider with 45 years of hospice expertise, we are committed to delivering the highest quality of care. In this role, you will assess the social and emotional needs of our patients and their families, develop appropriate care plans, and coordinate the necessary resources and services to ensure their comfort and well-being during their end-of-life journey.

Employment Status: Full Time or Part-Time

Pay: Starting at $26+/hour (pay is based on requirements and like experience)

Essential Duties & Responsibilities

  • Provide emotional and psychosocial support to patients and families
  • Collaborate with interdisciplinary healthcare teams for comprehensive patient care
  • Liaise with community organizations and agencies for additional patient support
  • Participate in agency activities, including team meetings and educational opportunities
  • Qualifications

Qualifications

Required:

  • Current unrestricted State of Kansas license to practice as a Licensed Master Social Worker.
  • Knowledge and skills in healthcare coordination, pain/symptom management, crisis intervention, and counseling.
  • Valid Kansas Driver's License, automobile liability insurance, and a Motor Vehicle Record (MVR) that meets organizational insurability standards as defined by Midland policy.

Preferred:

  • One year of professional medical-surgical nursing experience or hospice experience
