Make a difference as a LPN Patient Care Liaison

As a Full-Time LPN Patient Care Liaison at Inspired Homecare, your day-to-day responsibilities will include coordinating and overseeing patient care plans, communicating with healthcare providers, and ensuring that all patient needs are met promptly and effectively. Your role as a Care Coordinator will involve problem-solving to address any issues that may arise, demonstrating empathy and understanding towards patients and their families, and maintaining a high level of customer-centricity at all times. Integrity is key as you handle sensitive information and make critical decisions to provide the best possible care. Are you the LPN Patient Care Liaison we're looking for?

Knowledge and skills required for the position are:

• Experience in Healthcare Marketing

• Sales and Marketing and/or Admissions experience in healthcare with regard to generating referral sources

• Excellent communication and relationship-building skills

• Competitive nature

• Knowledge of area hospitals and other local healthcare organizations

• Has established relationships with social workers and discharge planners

• Good understanding of HIPAA regulations

• Good understanding of private insurance, Medicare and commercial insurance

• Local travel required (reimbursement provided)
