Job Summary:
The Registered Nurse (RN) Case manager will oversee aspects of patient care for a sub-set of home hospice patients. The RN Case Manager will ensure the delivery of quality patient care through professional assessment & observation, intervention, evaluation, and supervision of the entire interdisciplinary team and respective patient care plans.
Supervisor: DON (Director of Nursing)
Qualifications:
A minimum of 3 years nursing experience, hospice preferred. Previous case management experience preferred.
Ability to work independently.
Possess the ability to assist with and respond to the needs of terminally ill patients.
Ability to communicate well with others. In addition to excellent written/verbal communication, problem solving and decision-making abilities, successful candidates will also have demonstrated experience developing and maintaining a rapport with patients and families, and the ability to work well with an interdisciplinary team. Ability to work a flexible schedule. Successful candidates are compassionate with a strong work ethic. Frequent travel in and around the following counties: Dekalb, Gwinnette, Rockdale, Walton.
Responsibilities:
As an RN Case Manager, you will be managing client care with specific knowledge and experience in bedside care, symptom management, and crisis and family intervention. Additionally, ensuring that all clinical services provided are consistent with the hospice standards of care and comply with all written policies and procedures.
- Coordinates and supervises the overall provision of home care for hospice patients and their clinical care plans
- Ensures that clinical records are maintained in compliance with state, federal and agency regulations & policies
- Assist in the intake/admissions process as needed
- Serve as a consultant to each team member by providing clinical expertise and assistance as needed
- Facilitate home care visits with/by Medical Director as needed
- Facilitate all documentation needed for initial certification, re-certification, and any discharges from hospice
- Assess patient care issues through use of the CQI/PI
- Assists in providing administrative and on-call nursing support
- Assist in the development and implementation of policy and procedures related to clinical issues within scope of the clinical services provided
- Conducts supervisory visits in accordance with state and federal regulations
- Monitor and adjust the caseload of each clinical staff member and oversee the schedule of weekly visits
- Assist in orientation and staff training as needed
- Coordinate support from each IDG member as needed
- Ensures policy and procedures are in line with care plan
- Ensures effective communication for staff, patients, families & caregivers
- Participates in on-call rotation
- Assure that standards of ethical business and clinical practice are maintained
- Monitors and ensures compliance with Federal and State regulatory agencies and other certifying bodies.
- Reports grievances and complaints made by patients or families to the appropriate person(s).
- Reports all allegations of patient abuse and/or misappropriation of patient property.
- Reports all accidents and incidents. Identifies and responds appropriately to emergency situations.
- Observes and ensures safety needs of the patient are met
- Assist in the marketing of the hospice by making the company a community resource
- Perform other duties as assigned by the Administrator and Quality Assurance Director and DON

The noise level in the work environment is usually moderate. Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive patients.
