The Admissions Coordinator serves as the primary point of contact for all prospective and incoming clients. The Admissions Coordinator works closely with the outreach staff and Clinical leadership to assess patients and determine whether NorthSight Recovery services can meet their clinical and social determinate of health needs. The Admissions Coordinator or designee is responsible for coordinating client transportation for intake purposes, obtaining previous treatment records when appropriate, and providing the initial client orientation to NorthSight Recovery.

Job responsibilities include:

  • Serving as the organization’s first point of contact for prospective patients on the mainline.
  • Being readily available to respond to calls outside of regular business hours, if necessary.
  • In concert with clinical leadership, identify evidenced-based patient screening tools and/or develop agency-specific patient screening tools using evidence-based principles.
  • Conducting clinical and Transitional Living prescreen assessments, including the evaluation and review of assessment results with Clinical and Medical leadership.
  • Confirm prospective patient benefit information to determine eligibility for services. Communicate information clearly to internal stakeholders.
  • Utilization of the organization’s EMR to track and monitor referral sources and statistics.
  • Collection of all necessary documentation from patient and previous treatment providers prior to admission.
  • Identification and analysis of environmental and departmental trends and communication of those trends with the Chief Program Officer.
  • Facilitation of external referrals and the coordination of care.
  • Demonstration of an understanding of and adherence to all company policies and procedure.
  • Other related duties as may be assigned from time to time.


Qualifications:

  • Experience working in a co-occurring clinical inpatient or outpatient setting.
  • Ability to identify relevant clinical and medical issues that have a bearing admission criteria and patient care.
  • Excellent organizational skills and an eye for detail
  • Strong internal and external customer service skills.
  • Bachelor’s degree preferred, but will consider combination of relevant experience in lieu.
  • Minimum of two years’ experience in behavioral health field.
  • Supervisory experience
  • Previous clinical admissions experience preferred.
  • Ability to work within a multidisciplinary team and take direction from the clinical team.
