Position Summary

The Pharmacist is responsible for reviewing and interpreting physician orders, dispenses medications, monitors drug therapies, and provides accurate and timely drug information while complying with all legal and practice standards. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department personnel. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care as required.

General Duties, Tasks and Responsibilities

• Reviews prescription for accuracy in a timely manner; reviews and monitors drug therapy that requires dosing adjustments as authorized by provider.
• Interprets technical instructions and abstract variables; applies mathematical operations such as exponents and logarithms to dosage calculations, formulation of pharmaceutical preparations and data analysis (kinetics, TPC monitoring); participates in interdisciplinary conferences and planning for appropriate patient care.
• Provides information to providers, healthcare team and patients on the appropriate use of medications, including side effects, interactions, storage, dosage; monitors and maintains an up-to-date patient profile for changes in information including: allergies, interactions, and incompatibility; maintains patient profile and charge system.
• Checks all patient medications prior to delivery to patient for accuracy; compounds medications accurately using standard formulas and processes; reviews prescriptions issued by providers for accuracy and ability to complete; prepares or directs pharmacy staff in the preparations of IV admixtures and parenteral products using aseptic technique; supervises pharmacy support staff in the routine functions of the pharmacy.
• Ensures the maintenance and proper operation of equipment and supplies; maintains security of controlled substances and disposal of hazardous waste per protocol.
• Performs all aspects of the position in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
• Participates in performance improvement activities.
• Performs other duties as assigned.

Certification/Licensure Requirements

• Current IL Pharmacist License Required
• BLS Required
• ACLS Required within 1 year
• PALS Required within 18 months

Experience Requirements

• Pharmacy minimum 2 years Preferred
• Education in all age groups ranging from newborn to the geriatric population minimum 1 year Preferred
• Automated Pharmacy systems, IV Laminar Flow Hood minimum 1 year Preferred
• 340b experience Preferred

Computer Skills

• Strong computer skills including Microsoft Word, Excel and Outlook

Additional Skills

• Ability to work independently, prioritize and complete tasks in a timely manner
• Excellent communication and customer service skills
• Knowledge of state, federal, accreditation and regulatory agency laws and requirements
