Job DescriptionJob DescriptionAbout This Role:Join a mission-driven team that provides high-quality, integrated care to frail older adults. As a Registered Nurse (RN) for the Program of All-inclusive Care for the Elderly (PACE), you will be responsible for the management and delivery of direct nursing care to participants in various settings including clinics, adult day centers, homes, virtually, and skilled nursing facilities (SNF). You will be a critical Interdisciplinary Team (IDT) member, working collaboratively to complete assessments and develop participant care plans. This role allows you to practice at the top of your license and promote ongoing professional development.
  • Interdisciplinary Team (IDT) Participation: Contribute insights from assessments, care plan recommendations, and care coordination in a collaborative spirit.
  • Conduct Nursing Assessments: Perform face-to-face assessments inclusive of physical, psychosocial, and behavioral statuses in various settings (home, day center, virtual, long-term care facility).
  • Deliver Personalized Care: Partner with medical providers to deliver care for a panel of participants based on care plans.
  • Nursing Interventions: Document and deliver nursing interventions as agreed upon in care plans, respond to physician orders promptly, and correctly administer medications and therapeutic interventions.
  • Triage: Independently initiate therapies within the scope of practice and collaborate with medical providers to escalate care as needed.
  • Case Management: Provide case management during transitions of care and communicate proactively with hospital and long-term care facility case managers for care coordination.
  • Education: Educate participants, caregivers, and team members on personalizing and carrying out care plans.
  • After-Hours Calls: Remotely& take after-hours calls triaged by a nurse team on a rotating schedule.

Attributes of an Ideal Candidate:
  • Passion and mission orientation for serving high-risk seniors and frail older adults.
  • Minimum 2 years of experience clinically caring for medically complex or older adult populations preferred.
  • Strong active listening and conflict management skills.
  • Ability to adapt quickly to multiple clinical settings.
  • Strong clinical acumen in providing clinical interventions to complex participants.
  • Demonstrated experience in wound care, IVs, phlebotomy, colostomy/ileostomy care, etc.
  • Active, unrestricted RN license in California.
  • Graduate of an accredited School of Nursing, BSN strongly preferred.
  • CPR and First Aid certification (within 60 days of employment).
  • Alzheimer's certification is preferred but not required.
  • Bilingual in English and Spanish preferred.
  • Proof of current CPR/BLS certification is required or one must be obtained within 30 days of being hired.
  • Proof of a valid CA driver’s license, personal transportation, good driving record, and auto insurance as required by state law.
