Requirements:
  • Responsible for developing a comprehensive understanding of the core and support services provided by PIHC and the importance of patient retention in care.
  • Routinely reviews and utilizes patient-level data to identify PIHC patients with poor health outcomes and/or at-risk of falling out of care. Works closely with clinic, medical case management and behavioral health staffs to resolve patient barriers to care.
  • Must be able to communicate effectively with patients from diverse backgrounds.
  • Must be able to communicate effectively and efficiently with PIHC staff across all departments.
  • Works primarily at the Marietta location but may work at either PIHC Decatur or PIHC Duluth as assigned by supervisor.
  • Attends inter-agency meetings, workshops and required agency trainings.
  • Drive and maintain the agency van at the Duluth location as assigned by supervisor. Candidate must possess and demonstrate proof of holding a valid, GA Driver's license
  • Organizes and schedules all transportation requests for the Marietta location and assists with transportation requests at other PIHC centers as assigned by supervisor.
  • Able to utilize spreadsheets (Excel, SmartSheets) to maintain and monitor Federal, State and community grant requirements for transportation services.
  • Enters Transportation data in patients' Electronic Medical Records weekly.
  • Tracks and ensures that patients' Annual Non-Emergency Transportation waivers are in compliance.
  • Possess and demonstrate excellent customer service, interpersonal and organizational skills.
  • Tracks and records the purchase and disbursement of MARTA Breeze Cards and Quik Trip cards.
  • Manages the Transportation Pool in the agency's Electronic Medical Record (EMR) system.
  • Requires regular weekly travel between PIHC locations. Position is full-time (40 hours per week) and requires working a flexible schedule that will include working extended hours as assigned (Tuesday and Wednesday evenings; Saturdays).
  • Written and verbal proficiency in Spanish is desired but not required.
