SUMMARY: To manage NF/SNF residents and the RAI process in accordance with current Federal, State, and Local standards governing long-term care facilities, and as may be directed by the Director of Nursing to ensure that the highest degree quality care can be provided to the residents at all times.

  • Internal Relationships: Has daily contact with residents, staff members, resident family members and volunteers in completing their assigned job functions.
  • External Relationships: Physicians, other health care providers, and visitors.


ESSENTIAL JOB FUNCTIONS:

MDS

  • Assess and review charts of NF/SNF residents.
  • Lead team to assure real time participation in interdisciplinary care plan process.
  • Participates in interdisciplinary care plan meetings with interdisciplinary team per care schedule providing input regarding resident care needs and goals.
  • Review NF/SNF residents with appropriate Medicare/Insurance reviewers for current and previous residents.
  • Complete Comprehensive MDS, 5 Day and Quarterly assessments for weekly scheduled residents.
  • Schedule all weekly MDS list of all assessments as assigned and input data into provider matrix.
  • Generate and/or review all pertinent reports. E.g., Case mix reports and QI reports (both monthly)
  • Coordinate RAI process with interdisciplinary team.
  • Complete baseline care plan summary for new admissions.
  • Help IDT team update care plans quarterly/annually.
  • Review/Audit completion of MDS, CAA’s Care Plans, lock as required.
  • Transmit MDS assessments to CMS weekly.
  • Be familiar with State and Federal regulations in order to maintain survey compliance.
  • Be “On-call” on a rotation with nursing administration, as assigned.
  • Assist with compliance rounds.
  • Complete all necessary/mandated paperwork for NF/SNF discharges (NOMNC’s, SNFABN’s).
  • Manage Medicare certifications for providers to ensure signatures are completed according to CMS guidelines.
  • Help IDT team in the submission of continued stay requests to Medicare Advantage Insurance companies for authorization of Skilled Services.
  • Be an advocate of residents and family’s rights.
  • Provides ongoing education to staff regarding MDS requirements and regulations.
  • Participates in quality improvement activities related to MDS assessments and resident care.
  • Participate in daily stand-up meetings.
  • Facilitates/manages restorative nursing program in LTC cottage environment.
  • All other duties as assigned.


Infection Control

  • Participates in developing, implementing, maintaining, and periodically updating written policies, procedures, consistent with the Mission and Philosophies of St. John's United.
  • Participates in surveys and inspections by government agencies and make reports and recommendations to the Director of Nursing and Senior Administrative Officer concerning operations of the department.
  • Participates in developing, implementing, and maintaining a quality assurance/infection control program. Ensures excellence of care by enhancing continual quality assurance by daily contact through reporting, and communication with professional staff.
  • Develops quality improvement activities related to informatics and information technology and infection control within Nursing and the SJU organization resulting in improved information flow and improved quality outcomes.

  • Provide leadership and management in the skilled nursing environment tominimize infections and ensure staff follow proper precautions to prevent the spread of infection and incorporating appropriate measures to minimize exposure to infections. Incorporate advanced analysis, organizational, and problem-solving skills to effectively manage infections for all staff and residents.
  • Works with Director of Nursing to develop and review annual infection control plan.
  • Maintains line listing of all elder and staff infections.
  • Responsible for the implementation and review of the facility’s infection prevention program ensuring that existing standards and guidelines of applicable professional organizations and regulatory and governmental agencies are incorporated into the program.
  • Reviews relevant public health issues to integrate into practice, serves as a consultant and resource person regarding infection prevention issues for all facility personnel, collaborates with other healthcare professionals within and outside of the facility, and implements infection prevention education programs.
  • Systematically collects and analyzes healthcare-associated infection data using epidemiological principles and statistical methods to identify trends and risk factors.
  • Perform the reporting of HAI and quality data for the various facility, county, state, and federal reporting requirements.
  • All other duties as assigned.
Requirements:

REQUIREMENTS:

Knowledge and skills acquired through completion of State Certified, Registered Nurse Program. Must be licensed as a Registered Nurse in the State of Montana. Preferred 3 to 5 years’ experience directly related to duties and responsibilities specified. Must be licensed as RN in the State of Montana.

Knowledge, Skills, and Abilities

  • Knowledgeable of general, rehabilitative, and restorative nursing medical practices and procedures and laws, regulations and guidelines governing long-term care
  • Must have certification or be willing to obtain certification for Resident Assessment Coordination and infection preventionist training within the first year of employment.
  • Possess excellent communication skills both verbally and in writing.
  • Strong attention to detail and accuracy.
  • Possess excellent nursing assessment skills and general organizational skills.
  • Ability to interact effectively with residents, family members and co-workers.
  • Must show ability to read and write in a legible and understandable manner.
  • Must speak and understand English.
  • Computer skills or a willingness to learn.

Complexity/Difficulty

This position requires the ability to function independently in performing assigned duties. Must be able to remain calm under stressful and urgent situations. The person needs to be able to respond to elder’s needs in a timely and professional manner, recognizing the elders’ rights. Incumbent performs job tasks following established procedures and reports problems or concerns to supervisor.


Working Conditions

Normal health care environment. Requires lifting up to 10 pounds on a regular basis and up to 25 pounds occasionally. Bending, walking, and standing for prolonged periods of time.
