Job Summary:
We are currently seeking a compassionate and skilled Registered Nurse (RN) to join our Long Term Acute Care (LTAC) team. As an RN in LTAC, you will provide specialized nursing care to patients with complex medical conditions who require extended hospitalization. The ideal candidate will have previous experience in acute care or critical care nursing, strong clinical skills, and a commitment to delivering high-quality care to patients in a long-term acute care setting.

Responsibilities:

  • Assess patients' medical, surgical, and psychosocial needs upon admission and throughout their stay in the LTAC unit, collaborating with interdisciplinary team members to develop and implement individualized care plans.
  • Provide skilled nursing care, including medication administration, wound care, respiratory support, and monitoring of vital signs and patient condition, in accordance with physician orders and established protocols.
  • Monitor patients' response to treatment and changes in condition, communicating with the healthcare team to adjust care plans and interventions as needed.
  • Coordinate with physicians, specialists, therapists, and other healthcare providers to ensure comprehensive and coordinated care for patients with complex medical conditions.
  • Educate patients and families about their medical conditions, treatment options, and self-care techniques, empowering them to participate in their own care and make informed decisions.
  • Document all aspects of patient care accurately and in compliance with regulatory requirements, using electronic medical records systems.
  • Adhere to infection control protocols, safety guidelines, and organizational policies to maintain a safe and supportive environment for patients and staff.
  • Participate in quality improvement initiatives, staff education activities, and interdisciplinary team meetings to promote excellence in LTAC nursing practice.

Qualifications:

  • Valid Registered Nurse (RN) license in the state of practice.
  • Minimum of two years of clinical nursing experience, with experience in acute care or critical care nursing preferred.
  • Certification in Advanced Cardiac Life Support (ACLS) and Basic Life Support (BLS) required.
  • Strong assessment, communication, and critical thinking skills.
  • Ability to work effectively in a fast-paced and dynamic environment, managing multiple priorities and complex patient cases.
