Shelter Aftercare Case Manager

Tubman is looking for an aftercare shelter case manager who will assess the victim/survivor's needs and connect them to appropriate services that meet their needs to successfully transition from shelter to housing. The aftercare shelter case manager will implement services with a trauma-informed, strengths-based, and client-centered approach while assisting people to gain and maintain housing. This position also acts as 1 of 2 project coordinators for the agency’s Domestic Violence Housing First project, funded by the Office of Justice Programs. This funding is secured through December 2025, at which point the agency hopes to secure new funding to continue this work. The schedule is flexible to support client and program needs (generally Mon-Fri 10:00 am-6:30 pm) and must include at least one evening per week in the shelter until 8:00 pm or later. The position will have office space on the shelter floor but approximately 50% of work hours will take place in the community.

Responsibilities
Provides aftercare case management services to shelter clients who have moved into housing, maintaining consistent contact with all clients (75% of job).
- Creates safe and supportive relationships with clients using a strength based, non-judgmental and client centered approach.
- Provides resources to clients to support meaningful client outcomes leading to housing stability.
- Supports clients in setting and achieving client-led goals to foster hope and healing and maintain housing.
- Completes necessary paperwork, data entry, case notes, and administrative work related to client services.
- Provides transportation to clients for appointments and commitments related to goal areas.
- Supports shelter advocacy staff as needed with the crisis line, walk-in clients, security maintenance and client de-escalation.
- Upholds professional boundaries and client-centered approach.
- Assists in providing training, mentoring, and development of new staff, interns, and volunteers.
Acts as 1 of 2 Project Coordinators for the Office of Justice Program’s Domestic Violence Housing First project (25% of job).
- Deepens and maintains partnerships with relevant agencies, landlords, workforce development providers, housing legal providers, and other appropriate systems and supports that help clients access and maintain housing.
- Participates in DV Housing First state cohort meetings and related work outside of the meetings.
- Become a subject matter expert in DV Housing First and housing navigation for staff and clients.
- Regularly attends Tubman team meetings for staff who help survivors secure and sustain housing or prevent homelessness, and Continuum of Care and local homelessness planning meetings.
- Trains staff, with other Project Coordinators, in housing navigation, housing first, and housing stability resources.
- Partners with the Program Director, Program Manager, staff, and survivors to evaluate work.
Minimum Qualifications
- Education (minimum education level): Bachelor’s degree in a related field or 4 years equivalent applicable experience preferably in domestic violence services.
- Ability to pass a DHS fingerprint criminal background study as required by program licensing
- Regularly required to stoop, kneel, bend, extensive walking, standing, and lift to 50 pounds
Experience Level (experience required to perform at a fully competent level):
- 2+ years of direct service experience related to violence and homelessness, including domestic violence, sexual assault, and exploitation.
- Experience in implementing interventions to support complex trauma.
Essential knowledge, skills, and abilities (specific competencies for the position):
- Ability to welcome and engage a diverse group of stakeholders with a demonstrated commitment to diversity, equity, and inclusion.
- Understand and work from the agency’s set of values and philosophical approaches; including harm-reduction/housing first, trauma-informed care, violence as a learned behavior, client-centered and led work, etc.
- Have access to reliable transportation, a valid driver’s license and insurance, and maintain a good driving record
- Set and uphold professional boundaries.
- Knowledge of domestic violence-related issues and systems.
Compensation & Benefits
The salary for the position is $23.50 per hour depending on your experience.
