Requirements:- Bachelor of Science in Occupational Therapy.
- Current State License as an Occupational Therapist, Current driver’s license required.
- One year of experience as an Occupational Therapist preferred. Home health experience preferred
- Home care home base proficiency preferred
- Proficiency in therapy skills as per accepted standards of practice, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment.
- Reliable transportation and valid/current auto liability insurance.
- Conduct ongoing clinical assessments, including OASIS assessments, to identify patient/client needs and determine levels of functioning.
- Evaluate patients’ occupational performance in areas such as self-care, work, leisure, and rest.
- Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior.
- Evaluate physical, social, and cultural components of patients’ occupational environment.
- Facilitate direct therapeutic interventions to address ROM, strength, coordination, adaptive equipment needs, and environmental modifications.
- Develop, evaluate, and revise the plan of care in collaboration with patients, representatives, and caregivers.
- Document clinical notes, phone orders, and other records in a timely manner.
- Accept clinical assignments consistent with education and competence.
- Provide physician-ordered services, including patient/client, caregiver, and family counseling and education.
- Supervise and guide occupational therapy assistants and home health aides as appropriate.
- Apply infection control and standard precautions to prevent contamination and transmission of disease.
- Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.
- Work in patients’/clients’ homes under various conditions.
- Ability to perform heavy lifting, bending, pulling, pushing, and prolonged standing/walking.
- Capability to work under stressful conditions to meet deadlines and patient/client needs.
- Requires hand-eye coordination and manual dexterity.
