POSITION SUMMARY

Responsible for patient benefits and issues relating to the mental and social well-being of the patient.

SALARY RANGE

$34.50 - $49.50 / per hour DOE

HOURS

8 per Day / 5 Days per Week

CATEGORIES OF DUTIES

All Locations:

1. Provides patient evaluation and assessments.

2. Identifies patients’ financial and other benefit needs and assists patients in obtaining these benefits.

3. Advocates on behalf of the patient with the various public and social services programs in the community.

4. Identifies patients’ emotional support systems and assists in the development and refinement of such systems, as needed.

5. Provides patients with short-term counseling and crisis intervention and makes available to the patient, a system of care for on-going mental health needs.

6. Provides linkage with the existing, most appropriate community resources and provides for emotional support, as needed.

7. Participates regularly in staff training, staff meetings, and relevant community meetings.

8. Documents all patient and agency interactions, as well as assessments in patient charts, and assists with the preparation of reports by submitting relevant documentation.

9. Other duties as assigned by Supervisor, Director and/or Administration.

EDUCATION/LICENSES/CERTIFICATES

Master’s degree in Healing Arts (i.e. social work, psychology, public health, health administration, counseling), or an individual with similar qualifications approved by the Department of Health Services, Office of AIDS, and/or OAPP.

HIV SPECFIC:

Knowledge of HIV resources and HIV-related clinical issues. Must have a valid California driver’s license and insurance.
