Are you the Registered Nurse we're looking for?To thrive as a Registered Nurse at Comfort Home Care, you'll need a blend of skills that not only enhance your nursing practice but also ensure exceptional home care delivery. Your ability to perform initial and ongoing assessments is crucial for identifying clients' medical needs and their communication capabilities. Initiating and revising plans of care, alongside executing OASIS assessments, will keep you at the forefront of patient-focused care. Collaboration is key-coordinating with the physician, family, and other healthcare providers ensures we're all on the same page for the patient's well-being.
Additionally, effective nursing interventions and timely preparation of clinical notes are essential for maintaining a seamless workflow. If you're a smart problem solver with a customer-centric mindset, we'd love to see how you can elevate our holistic approach to home care
Knowledge and skills required for the position are:
- RN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
- RN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
- RN initiates and performs appropriate nursing interventions.
- RN coordinates services informs the physician and other personnel of changes in the client's condition and needs.
- RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
- RN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
