Location | Department: Thome PACE | Jackson
Schedule: Monday – Friday | 8:00am – 4:30pm | back-up on-call (4.3 days a year) obligation that will include weekdays, weekends and holidays.
Overview:
Thome PACE® serves Jackson, Lenawee, and Hillsdale counties providing participants a wide range of medical and supportive services in our Day Clinic located in Jackson, Michigan or in the comfort of their own homes. The Thome PACE team of healthcare professionals specialize in working with the senior community to help participants and their family members make positive healthcare decisions and receive the care they deserve.
The Clinic RN is responsible for providing direct and indirect nursing care to PACE participants. This position works under the supervision of the Nurse Manager or designee but is independent in the application of advanced nursing knowledge and manages complex clinical situations.
What you’ll do:
Assessment:
- Collects patient data and completes required forms.
- Acts as backup for Triage Nurse.
- Performs fall assessments in collaboration with PT.
- Completes initial, semi-annual and annual assessments prior to the scheduled team meeting.
- Identifies participant’s problems/needs; sets priorities & further referral and/or follow-up.
- Interprets and records latest diagnostic results; performs advanced nursing assessment.
- Completes in-home assessments of prospective and current participants.
Care Planning:
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning.
- Develops a plan of care based on nursing process and which incorporates the plans of other disciplines and continuing care needs.
- Includes the patient/family in developing or revising the plan of care.
- Makes referrals to interdisciplinary support services.
Participant Care:
- Provides treatments/procedures and other care including but not limited to IV therapy, tube feeding, and wound care.
- Provides medication management, communicates needs, and follows up with pharmacy.
- Provides medication education.
- Provides participant and family education in coordination of 24-hour care delivery.
- Identifies emergency situations and independently initiates advanced therapy.
- Acts as coordinator of participant care with other health care personnel and evaluates patient care measures instituted.
- Ensures that the medication room is properly stocked and maintains logs as necessary.
- Tracks hospitalizations: completes admission and discharge responsibilities.
- Records care delivered and any variation to plan and reports and records new infections.
Leadership
- Attends clinic meetings and huddles.
- Using nursing process, delegates nursing care to appropriate personnel.
- Attends Nursing Facility Care Conferences as assigned
- Integrates cost effective measures into nursing practice.
- Recognizes unit problems and takes responsibility for documenting same.
- Demonstrates active participation in QI processes.
- Recognizes inappropriate and/or ineffective patient care management, resolves issue/problem and completes written reports.
What it takes:
- Graduate of an accredited School of Nursing.
- Licensed Registered Nurse in the State of Michigan.
- Must be CPR certified
- Must have three (3) years’ experience as a practicing nurse in a health-related area (Desired)
- Minimum of one (1) year working with the frail and elderly (Desired)
