Job Description :

  • Client is seeking a dedicated Physical Therapist to join our team full-time in Wall Township, NJ.
  • This on-site role involves providing evaluations, rehabilitation services, and facilitating transitions to home, Skilled Nursing, Independent Living, Assisted Living, or Memory Care neighborhoods at discharge.
  • Responsibilities include developing and implementing therapy programs, conducting assessments, and collaborating with the therapy team and facility staff.

Qualifications

  • Experience in Skilled Nursing Facilities (SNF), Assisted Living Facilities (ALF), Independent Living Facilities (ILF), and Memory Care.
  • Knowledge of medical and healthcare protocols.
  • Strong interpersonal and communication skills.
  • Ability to collaborate with a multidisciplinary team.
  • Active Physical Therapist license in the state of New Jersey.
  • Professional and friendly personality.
  • Commitment to providing top care and respect for the geriatric population.
