Role Overview: 

Independent Living (IL) Coordinators are dedicated professionals responsible for supporting and coordinating independent living services for consumers. They work directly with individuals with disabilities to help them achieve greater independence, enhance their quality of life, and participate fully in their communities. IL Coordinators assess consumer needs, develop individualized service plans, and provide ongoing support and advocacy.

Key Responsibilities:

  1. Consumer Assessment and Service Planning:
    • Conduct comprehensive assessments to determine the needs, goals, and strengths of consumers.
    • Develop individualized service plans (ISPs) that are person-centered and focused on promoting independence.
    • Collaborate with consumers to set realistic and achievable goals.
    • Review and update ISPs regularly to reflect changes in consumer needs and progress.
  2. Direct Support and Guidance:
    • Provide one-on-one support to consumers in various aspects of independent living, including daily living skills, financial management, and accessing community resources.
    • Assist consumers in navigating and applying for benefits and services such as housing, transportation, and healthcare.

  6. Crisis Intervention and Support:

Skills and Qualifications:

  • Education and Experience:
    • Bachelor’s degree in social work, rehabilitation, psychology, or a related field preferred. A high school diploma with appropriate work experience may be used as a supplement. 
    • Minimum of 2-3 years of experience in social services, rehabilitation, or related field.
    • Knowledge of independent living philosophy and practices.
  • Communication and Interpersonal:
    • Excellent communication and interpersonal skills.
    • Ability to build strong, trusting relationships with consumers.
    • Strong advocacy skills and a commitment to consumer rights.
    • Ability to work effectively with diverse populations.

    • Analytical skills to assess consumer needs and evaluate program effectiveness.
    • Proficiency in using case management software and other relevant tools.

    • Basic knowledge of assistive technology and its applications in independent living.
    • Ability to use office software and technology for documentation and communication.

Work Environment:

  • IL Coordinators typically work in an office setting but may need to travel to meet with consumers in their homes, community settings, or other locations.
  • The role may require flexible working hours, including occasional evenings or weekends, to meet the needs of consumers and the organization.
