POSITION SUMMARY

To help clients learn to manage their health care needs. Coordinate medical clinics for clients and use crisis intervention skills to help clients maintain recovery while navigating their health-care related issues. Track client interactions and services and report to appropriate departments. Monitor, maintain, and track clients self-administering controlled medications.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Educate and empower clients to navigate the current health care system.
  • Refer clients with health care issues to the appropriate facility to be evaluated and treated.
  • Track and maintain supplies, medications, and client services.
  • Facilitate classes on the Men's and Women's Campus.
  • Facilitate relationships with community partners and health services volunteers.
  • Cooperate with other departments regarding client healthcare issues when appropriate.
  • Clear and dispose of items in amnesty boxes on the Women's and Men's Campus
  • Facilitate controlled medication calls during shift as determined by Health Services Manager.
  • Track and report clients who do not show up for controlled medication calls to the Health Services Manager.
  • Data entry into Microsoft Access and Electronic Health Record.


QUALIFICATIONS / REQUIREMENTS

  • Minimum education requirement: High School Diploma or GED equivalent
  • Proficiency in Microsoft Office Suite
  • Valid Driver's License
  • Highly organized, motivated
  • Creative problem solver
  • Critical thinking skills
  • Teachable mindset
  • Access to reliable transportation
