Job Description for a Pharmacy Technician in Care Management:Key Responsibilities:
  1. Medication Reconciliation:

    • Assist in reviewing patients' medication histories and ensuring that all medications are accurately documented.
    • Identify discrepancies in medication lists and collaborate with pharmacists to resolve them.
  2. Patient Education and Support:

    • Provide patients with information about their medications, including how to take them, potential side effects, and the importance of adherence.
    • Answer patient questions or direct them to the appropriate healthcare provider for more complex inquiries.
  3. Medication Adherence Monitoring:

    • Track and monitor patients' medication adherence using electronic health records (EHR) and other tools.
    • Follow up with patients who miss doses or are at risk of non-adherence and report concerns to the care team.
  4. Care Coordination:

    • Communicate with other healthcare providers to ensure seamless care transitions, particularly when patients are discharged from hospitals or change care settings.

Skills and Qualifications:
  • Certification: Current certification as a Pharmacy Technician (CPhT) from a recognized certifying body (e.g., PTCB or NHA).
  • Experience: Previous experience in a pharmacy setting, preferably in care management environment.
  • Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and chronic disease management.
  • Technology Proficiency: Familiarity with electronic health records (EHR) systems, pharmacy software, and telehealth tools.
  • Communication Skills: Excellent verbal and written communication skills to interact effectively with patients and healthcare providers.
  • Attention to Detail: High level of accuracy in documenting patient information and processing medication orders.
  • Problem-Solving Abilities: Ability to identify potential medication-related issues and work collaboratively to resolve them.
