Department: Primary Care - Montour Falls, NY

Job Title: Licensed Practical Nurse

Position Status: Full Time, 40 hours per week

Reports to: Practice Manager, Primary Care, Women's Health, and Specialty Services

Job Summary: Licensed Practical Nurse assists providers in the provision of medical care to all patients in the outpatient clinic setting. The licensed practical nurse is responsible for rooming patients, assisting the provider during procedures, administering medications including but not limited to vaccinations and therapeutic injections, preparing charts, and completing care coordination functions. The licensed practical nurse, within their scope of practice will work under the supervision of a provider and/or registered nurse.

Job Responsibilities:

  • Rooms patients including taking a complete set of vitals, performing medication reconciliation, and completing all other necessary fields within the EMR for the type of office visit listed

  • Assists the provider during their examination and treatment of the patient, including but not limited to minor surgical procedures while using proper technique

  • Understands and administers medications based on correct use, correct dosage, and correct route for specific outpatient clinic setting, documents appropriately

  • Understands and performs nurse specific procedures based on clinic guidelines and successful demonstration of competency

  • Performs point of care lab testing within policy guidelines and documents results appropriately

  • Performs care coordination functions in a timely manner including but not limited to handling patient calls, managing communication between patients, providers, and other departments, managing incoming and outgoing office communication regarding patient care, etc.

  • Maintains integrity of all office equipment and supplies, including performing necessary infection control measures, stocking patient rooms and managing inventory (expiration dates, etc.)

  • Escalates complex patient care issues to registered nurse and/or provider in a timely manner

  • Attends departmental meetings, keeps up to date on office communication, and completes all assigned trainings

  • Performs other duties as assigned within the licensed practical nurse scope of practice

Requirements:

  • Education: Graduate of an accredited nursing school

  • Experience: Experience working in a clinic setting preferred, but not required; experience working in an electronic medical record preferred

  • Licensure: Current LPN licensure in the State of New York; Current BLS/CPR certification, if not current, will need to obtain within 90 working days

  • Skills: able to communicate in English, both verbally and in writing; basic computer knowledge required

  • Physical requirements:
    • repetitive use of hands and fingers (ex: use of computer keyboard),
    • may require lifting and carrying light loads (up to 20 pounds), including boxes, supplies
    • sitting, walking, or standing for long periods of time;
    • must be able to physically perform CPR/BLS measures
    • must be able to physically operate the equipment used for the job
