Position Summary:

NOTE: This position is a temporary role with a duration of approximately three (3) to six (6) months.

The temporary Onboarding Specialist is a community-invested, clinically focused professionally responsible for the outreach and onboarding of new patients onto GrayHawk Health's programs. The Onboarding Specialist is tasked with increasing participation in new programs through education, promoting, encouraging, and disseminating information while using creative recruiting efforts during telephonic outreach. During the initial contact, the Contractor Onboarding Specialist ensures that the potential patient has a full understanding of the services being offered and the benefits of those services.

The temporary Onboarding Specialist is highly motivated and committed to helping patients by connecting them to services that improve health outcomes. The Onboarding Specialist can clearly and concisely explain the program or service being offered and answer all questions so the patient or responsible party can make a well-informed decision about the acceptance of services or participation in a program offered through their health plan in conjunction with GrayHawk Health. Our organization is designed to improve health outcomes by ensuring that seniors have the necessary clinical and social support enabling them to thrive in place in the community.

Duties include but are not limited to:

  • Provide initial telephonic outreach to patients, carefully articulating GrayHawk Health's mission for patient empowerment and betterment.
  • Effectively communicate GrayHawk Health's full offering of clinical and social services.
  • Perform duties in support of GrayHawk Health's focus on ensuring that the highest quality of patient engagement is successfully provided for each patient touchpoint.
  • Coordinate with the clinical team to set up in-home and telehealth appointments.

Accountabilities/Criteria:

  • Daily telephonic outreach to 75 – 100 patient referrals per day
  • Education of patients on program benefits and patient responsibilities while enrolled in the program
  • Documents consent and schedules appointments in HER
  • Follows health plan requirements and protocols
  • Maintains a positive demeanor and good customer service with patients
  • Responsible for maintaining a high patient appointment success rate
