Job Overview:
The Registered Nurse Care Coordinator/Case Manager specializing in Genetics will play a pivotal role in managing and coordinating patient care within a genetic setting. The primary responsibilities include triaging incoming consults, managing patient communications, maintaining accurate records, and ensuring compliance with medication policies. The role also involves providing discharge education and follow-up instructions, particularly for Hematology/Oncology clinical patients.
Experience Requirements:
- Ensure all incoming consultations are triaged within two business days of receipt.
- Contact patients with negative results within three business days of when they are returned.
- Maintain a professional demeanor with no more than one substantiated complaint by patients or staff.
- Update the patient tracker with relevant information, ensuring minimal errors.
- Ensure that consultations to any other service are input and read for signature.
- Adhere to established medication policies and procedures. Note that sample medications shall not be provided to patients.
- Provide comprehensive discharge education and follow-up instructions coordinated with the next care setting for all Hematology/Oncology clinical patients.
- Ability to maintain a high level of accuracy and attention to detail.
- Strong communication and organizational skills.
- Commitment to adhering to established protocols and providing exceptional patient care.

- Graduate of a professional nursing program approved by the appropriate State agency and accredited by a recognized accrediting body.
- Current, full, active, and unrestricted registration as a registered nurse (RN) in a U.S. State, Territory, Commonwealth (including Puerto Rico), or the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment
- Current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS), or equivalency.
