Peaceful Living is seeking a Case Manager to Oversee the provision of Home and Community Based Services to the individuals on their caseload in accordance with legal requirements and Peaceful Living's philosophy of care.


Essential Job Functions:

  • Directly oversee a caseload of individuals; Complete regulatory requirements such as compiling progress notes, monitoring service utilization, and attending ISP meetings
  • Maintains professionalism when interacting with individuals and families who are under significant stress that they may not express constructively
  • Oversee Direct Support Staff while fostering their professional development, maintain clear communication on job duties and scheduling, and meet for regular 1:1 supervisory meetings
  • Maintain schedules for assigned caseload and share On Call duties with other Case Managers
  • Assist manager by processing referrals, participating in weekly Team meetings, and developing departmental policies, procedures, and initiatives
  • Provided direct support to individuals as available, in order to maintain service ratios and foster ground level understanding of individual programming and ensure quality care and support
  • Take self-initiative on learning best practices and skills related to field
  • Maintain familiarity with the policies, individuals, and systems of Family Services in order to provide support to the other Case Managers and the department

Education/Experience:

High school diploma or equivalent required. Associates degree or bachelor's degree in related field preferred; a minimum of one year of experience working within the field of intellectual and developmental disabilities
