Position Title: HVRP Case Manager Location: Newark, New Jersey


Reports To: Director of HVRP

General Purpose:

Case Manager must deliver comprehensive vocational case management, counseling and referral services to
the clientele (Homeless and at-Risk Veterans) transitioning from public assistance to employment, and to assist in attaining career goals that can lead to self-sufficiency. Must maintain the highest standard of personal conduct and integrity while working with the clientele, as well as
in relating to other staff members, service providers and agencies.

Essential Functions:

Performs weekly/daily initial assessments for clients and ensures
that clients have all necessary documents for employment such as valid proof
of Citizenship/US residency, Work authorization, Social Security Card, DD214. Self-Attestation form valid photo identification, educational and training documents and updates
resume.

Perform initial assessment and intake, including ensuring eligibility for the HVRP program, gathering documentation and creating the participant case file.

Conducts initial assessments as well as all follow up meetings to assist
clients with job search outcomes. Schedules case manager appointments and completes case notes and assessments adhering to time frames set by
HRA and Goodwill Industries.

In partnership with the Career Specialist conduct initial career exploration and assessments to inform the individualized service plan.

Provides referrals, follow-up counseling, resource management and guidance services to address barriers to employment and make appropriate referrals to vendor services, agencies,
and Partner Organizations as required to help overcome these barriers and
better prepare clients for employment opportunities. Barries can include homelessness and food insecurities.

Utilize information gathered in the intake to provide support service referrals (including housing, food, clothing). Maintain coordinated case management with partner organizations to ensure services are responsive to participants needs.

Other regular responsibilities:


Provides outreach to clients at Veteran Shelters, Hospitals, Veteran Programs and Events. Outreach may also include meetings with potential participants in the field who have transportation concerns. Outreach may include occasional weekends.

Provide outreach to Veteran’s serving organizations to engage potential participants and staff to identify and connect Veteran’s to HVRP and other related services.


Introduces program rules, policies, regulations, time limits, assessment
process, program flow, job development and retention.

Conduct note taking about periodic participant counseling check ins on footnotes program.

Document all client services and referrals in case notes, support service trackers etc. to ensure all participant related activities are tracked and in compliance with Goodwill NYNJ and HVRP standards.

Submits weekly report to HVRP Director with updates on case
notes, clients serviced, referrals for training and specialized services,
infractions posted, resource management and career preparation.

Work as part of an inter-disciplinary team to ensure Veterans receive coordinated services that connect them to employment and retain in those positions.

Qualifications/Basic Job Requirements:

BA degree required with two years related working experience with client
population.

Two or more years of related case management experience.

Knowledge of local Veteran Community, labor market, industries and employer hiring practices.

Experience working with Military Veterans and the Military Veteran Community, Military Veterans are highly encouraged to apply.

Must have excellent organizational skills, strong interpersonal and
effective problem-solving skills. Excellent motivational and presentation skills.

Exercises discretion, judgment, and confidentiality.

Maintains security of customer information and files.

Bilingual preferred.

•Must be computer literate.

Ensures compliance with Agency policies and procedures, and safety
standards throughout department work areas.

Must be flexible and able to respond to the changing needs of the
program.
