Position Summary:

The Behavioral Health Clinician represents Roots Community Health Center, working as part of a team in a highly visible setting. They will see Roots clients individually and in group settings. If an intern, they will be supervised accordingly as directed by training and licensing guidelines.

Duties and Responsibilities:

  • Provide therapeutic services to patients/clients and/or their families with complex bio-psycho-social adjustment issues.
  • Develop with patients/clients and/or families appropriate intervention plans based on careful assessment of bio-psycho-social factors in the situation.
  • Provide individual and group therapy.
  • Conduct behavioral health assessments and intakes.
  • Document all treatment provided in a thorough and consistent manner.
  • Provide trauma-informed intervention and services to individual patients/clients who are coping with temporary or permanent disability, critical illness, or terminal illness.
  • Work with Navigators to assist patient/client and/or family on the implementation of plans with independence and initiative.
  • Work collaboratively with health care staff in the multifaceted management of bio-psycho-social needs and issues.
  • Interpret relevant factors to other members of the care team.
  • Provide information to community upon request about resources and services available through the Roots Community Health Center.
  • Provide safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care.
  • Work with members served and multidisciplinary treatment teams to design, implement, and evaluate the Recovery Plan.
  • Under clinical supervision, establish and maintain a therapeutic relationship with members served.
  • Ensure provision of needed clinical services to assigned members served.
  • Coordinate ongoing treatment within the program and with outside agencies.
  • Collaborate with other staff in the discharge planning process and makes linkages to community services for members served.
  • Actively participate in multidisciplinary team meetings and recovery planning meetings.
  • Demonstrate knowledge of de-escalation techniques and crisis communication and management.
  • Serve as a clinical consultant to recovery team members.
  • Demonstrate the Roots’ mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders.
  • Attend department and organization meetings as required.

Competencies:

  • Master’s degree in Counseling, Psychology, Social Work, or related field.
  • LCSW, LPC, LMFT, PsyD, and PhD, must hold license in the State of California.
  • ASW’s and AMFT’s will receive supervision towards licensure.
  • Experience with evidence-based practices, including trauma-informed care framework.
  • Knowledge and experience working with communities of color that are experiencing co-occurring issues, such as multiple chronic health conditions, mental/behavioral health and substance addiction.
  • Interest in working with the entire spectrum of mild-moderate and moderate-severe behavioral conditions, as well as working with formerly incarcerated and unsheltered individuals.
  • Ability to complete social histories, risk assessments, mental health status exams, and plans of care.
  • Ability to work collaboratively with physicians, nurses, and rehabilitation therapists
  • Ability to form a therapeutic alliance with members served and families.
  • Sufficient medical knowledge to assess the needs of members served.
  • Skilled in conducting group therapy.
  • Ability to advocate for the members served in other health delivery systems.
  • Active listener with strong data entry skills.
