The Home Care Clinical Manager is responsible for mentoring, monitoring, tracking and coordinating clinical activities. Manages the communication and coordination of quality services of the multi-disciplinary team approach to safe and exceptional patient/client care.


Essential Functions:

1. Provides expert guidance and assistance to the DPCS in all matters relating to the delivery of services to assure that quality standards are maintained. Demonstrates management skills needed to orient, supervise and coordinate staff, establishing appropriate accountabilities.

2. Performs duties necessary to maintain quality nursing standards.

3. Triage calls, process and coordinate doctor’s orders as appropriate; follow-up for documentation of verbal orders; ensure the timely delivery of information received to appropriate clinicians.

4. Process requests for service (referrals), as needed, ensures all information is complete and accurate and forward to the appropriate personnel.

5. Reviews, coordinates and follows-up on 485’s, OASIS, clinical notes and DARs and ensures the documentation required for the reimbursement of care is completed on a timely basis and in compliance with state and federal regulations.

6. Coordinate staffing needs for contracted professional nursing staff on an as needed basis, determine through analysis of staffing trends and needs.

7. Works with the education staff in providing in-service programs to all nursing staff. Provides adequate opportunity for education and maintain safety and standards of quality care.

8. Represent the agency at various committees in the agency and community as well as industry specific conferences, meetings, seminars etc.

9. Perform periodic performance reviews on all nursing staff in order to ensure that the staff are meeting or exceeding job standards.

10. Peforms or assigns qualified professional on a periodic basis to perform on-site evaluation of staff in order to assess the maintenance of safe standards of quality care.

Non-Essential Functions:


  1. Keeps abreast of home health and management issues.
  2. Assures agency compliance with all federal, state and local laws as well as adhering to the Norwell Visiting Nurse Association, Inc. Corporate Compliance policy in the prevention of fraud and abuse on behalf of the Agency.
  3. Performs other responsibilities as assigned.

Skills/Abilities:

    • Strong clinical skills
    • Strong communications skills, both verbal and written
    • Ability to work under pressure
    • Ability to be detail oriented and follow precise directions
    • Strong organizational skills, decision-making, problem solving and time management skills
    • Ability to be flexible and dependable and embrace change and new technology

Education:

    • Current professional license to practice as an RN or PT, OT, SLP in Massachusetts (in good standing)
    • Bachelor’s Degree required or equivalent experience

Experience:

    • Two (2) plus years’ community health experience (minimum)
    • Two (2) years of supervisory experience preferred
