Position: Home Care Nurse
Location: Wall Township, NJ
Duration: 3 Months

Job Summary:

The Home Care Nurse is responsible for delivering professional nursing care to patients within the comfort of their homes. This role involves assessing, developing, implementing, and evaluating individualized home nursing care plans for assigned patients. The Home Care Nurse ensures compliance with physician orders, documentation of patient progress, and adherence to organizational policies and external regulations. Collaboration with interdisciplinary teams, accurate documentation, and maintaining safe patient care are essential aspects of this role.

Key Responsibilities:

  • Provide professional nursing care to patients in their homes, following established care plans and physician orders.
  • Ensure compliance with physician orders and visit frequency according to established guidelines.
  • Document patient care activities accurately and in compliance with organizational standards and regulations.
  • Utilize critical thinking and professional standards of care to achieve safe and effective patient outcomes.
  • Coordinate services effectively to meet patient needs and ensure appropriate resource referrals.
  • Act as a liaison among physicians, clinical management, and other healthcare team members to facilitate effective communication and coordination of care.
  • Supervise para-professional home health aide staff and participate in their evaluation.
  • Stay updated on new standards and guidelines in nursing practice and incorporate them into patient care.
  • Actively participate in the evaluation and improvement of patient care processes.

RN Responsibilities:

  • Provide nursing care to patients per MD orders, including wound care, foley catheter care, and patient education on diagnoses, medications, and related topics.

Required Skills & Experience:

  • Minimum of 6 months to a year of home care experience.
  • Knowledge of OASIS and experience with Wound Care, Wound VAC, Tubes, Drains, and Infusion.
  • Excellent customer service and communication skills.
  • Competency in completing assigned treatments and procedures according to standards and policies.
  • Ability to care for patients and significant others in a holistic manner.
  • Two years of relevant nursing experience, including acute care/hospital experience.
  • Adaptability, accountability, teamwork, dedication to ongoing education, and a strong focus on patient care.

Preferred Skills & Experience:

  • Knowledge of Home Care EPIC application.
  • Certification in Home Care strongly preferred.
  • Previous and recent IV experience within the last year.
  • Prior hospital experience.
  • Medicare certified home care experience is highly preferred.

Additional Information:

  • RNs are responsible for driving themselves to and from patient homes in Monmouth County. Mileage is NOT reimbursed.
