POSITION TITLE: Community Services Coordinator  

DIVISION:  Community HAB  

SUPERVISOR: Regional Director of Community Services  

CLASSIFICATION: Non-Exempt  

PURPOSE:    

Our agency strongly believes that every individual deserves the opportunity to achieve their fullest potential, including employment, continuing education, and community participation. Coordinators play a key role in helping clients achieve their goals. The right candidate will mentor and provide compassionate support to clients with intellectual and developmental disabilities, while working to further the agency mission.

KEY AREAS OF RESPONSIBILITY:

  1. Develop Community Integration and Education schedule and class curriculum.
  2. Ensure direct service staff follows schedule and curriculum.
  3. Arrange for community locations for classes to be held. Ensure integration with community members.
  4. Network with coworkers, the community, and other professionals to find community opportunities for clients.
  5. Ensure that necessary supplies are obtained and kept in good condition.
  6. Provide information regarding staffing needs to Director of Community Services and HR.
  7. Complete required training, including CPR, First Aid, NCI, and ongoing professional development.
  8. Completes other duties as assigned.
  9. Maintain good driving record, current driver’s license, and transportation.
  10. Assist the QIDPs with staff direction to ensure PCISP (Person-Centered Individualized Support Plan) requirements are met, as needed.
  11. Ensure employees dress appropriately as defined by program dress code and activity consistent with job requirements and using Agency-provided personal protective equipment as appropriate.
  12. Work with E & E Coordinator to ensure appropriate curriculum is developed and implemented.
  13. Assist with grant opportunities.
  14. Maintain positive budget and monitor spending of agency funds.

REQUIREMENTS:

  1. High School diploma or GED required, Bachelor’s degree in Psychology, Social Work, Special Education, Sociology, or related fields a plus.
  2. Experience in providing services to individuals with intellectual or developmental disabilities preferred.
  3. Must have excellent oral and written communication skills and maintain confidentiality with sensitive information.
  4. Maintain a valid Indiana driver’s license, insurance on personal vehicle (minimum of PLPD with medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
