Midlevel Provider

Job Description: Under limited supervision provides  medical services to patients emphasizing health promotion and disease prevention. Manages acute and chronic diseases in accordance with medical protocols. Assesses the patient’s condition, interprets laboratory tests, prescribes appropriate treatments/therapies and educates patients.

  • Performs comprehensive assessment of patients' health care needs 
      1. Uses appropriate interview techniques to obtain pertinent health, family and/or sexual history.
      2. Performs complete, systematic physical and/or psychosocial assessments of patients.
      3. Performs and/or orders appropriate laboratory studies, analyzes results and develops diagnosis based on findings.
  • Develops comprehensive plan of care for patients
      1. Accurately analyzes and evaluates findings from health histories, laboratory studies and physical examinations.
      2. Develops a comprehensive plan that addresses identified problems and needs appropriately and incorporates all appropriate resources.
      3. Completes plan of care within specified time frame.
      4. Modifies plan of care as indicated by reassessment of patient's needs.
      5. Collaborates effectively with patients/families, physicians, other team members and other skilled individuals in the development of a plan of care.
  • Provides treatments and administers medications; provides preventive and maintenance care to patients/families consistent with care plan and/or physician treatment plan
      1. Follows established policies, protocols and/or physician orders in performing treatments and administering medications..
      2. Ensures that the patient receives follow up care as appropriate.
      3. Refers patient to other health care givers as deemed appropriate
  • Provides health education and instruction to individuals, families and classes for promotion and maintenance of health
      1. Provides accurate and appropriate information.
      2. Educate patients for diet, activity, hygiene, and disease prevention
  • Provides acute/episodic health care
      1. Identifies acute problems/conditions accurately.
      2. Determines if a patient needs referral to a physician for a specific acute condition.
      3. Utilizes resources for appropriate referral for hospitalization or emergency room.
      4. Able to work with limited resources and personnel
      5. Able to belong to and utilize a team in providing quality health care to our patients
  • Provides appropriate follow-up care 
      1. Performs reassessment of patient when appropriate to determine the outcome of treatment and the resolution of the condition.
      2. Implements or ensures implementation of appropriate plan of care.
      3. When necessary, consults with a physician on plan of care
  • Ensures quality assurance standards of care are met in programmatic areas 
  1. Provides appropriate documentation.
  2. Utilizes flow sheets and other forms according to protocol
  • Prioritizes continuing education in order to ensure best practice 
    1. Maintain Board Certification requirements
    2. Maintain required skills i.e. CPR and ACLS
    3. Maintains Family/Adult practice medical skills that meet the needs of Partnership Health Center
    4. Actively engages in obtaining the knowledge and technical skills to serve special-need populations i.e. diabetic, hypertension, etc


Minimum Qualifications: 

Current & active certification as a Certified Physician Assistant or Nurse Practitioner to practice in the state of Georgia without limitations. 5+ years of experience preferred.
