Hospitals Case Manager

Pay Rate: $18 per hour depending on education and experience
Education Requirements:
A Bachelor’s degree in human service field (i.e., social work, education, sociology, psychology, counseling, applied behavioral sciences) or criminal justice preferred.
Licensure/Certification:
Valid driver’s license and auto insurance
Experience Requirements:
A minimum of two years’ experience working in case management, with at least one of those years of experience working with economically disadvantaged, vulnerable or at-risk youth and/or adults.
The Case Manager for Camber Children’s Mental Health (CCMH) is responsible for management of clients’ care and will participate on a multi-disciplinary internal staff team, while interfacing with external stakeholders which include other care providers and insurance companies. The Case Manager coordinates parental involvement and communication as well as linkages to community resources. Shift available is Monday-Friday from 8am-5pm.
